It probably depends where you live and how much of a backlog there is in your area. I'm in Scotland and waited around 4 months for my appointment , that was nearly 2 years ago now.
I changed over last year and I seem to remember I had to ring for the form to be sent. They took quite a lot of details over the phone and then the form arrived in the post a few days later. You do have limited time to send the form back so I would start making some notes now so you have a head start when it arrives. Give them a ring and they will send the form. It is really important that you start preparing now to fill in the form; it is much harder to claim successfully than DLA. Keep a daily diary of your needs, what you need assistance for, where you struggle, what aids you use, start collecting together medical reports. Check out the NRAS web site, they have good advice about claiming. I paid a years member ship for the work and pensions group advice booklets, they are brilliant. They have some free information but it is worth becoming a member so that you can access there full information. It is also worthwhile having someone from welfare rights or CAB check over your form in case you have omitted anything.
