Concerned Husband - Proteinuria

Hi,

My 70-year-old wife is getting some test results that are very concerning to me. Initially she was referred to an oncologist regarding possibility of multiple myeloma. Praise God that came out negative. However, in both a random Urine panel and a 24-hr. test, her protein levels are very high. We're currently trying to get in to see a nephrologist. I'm a little freaked out.

Her protein/creatinine ratio was 930 mg/g. Her total protein was 130. She has frothy pee and swelling in ankles. Her serum eGFR was 64. Also, her B2M was 4.49 and her Free Kappa Lambda was 39.8.

From experience is this a dire situation for her regarding her kidneys?

I'm new, so please excuse my ignorance.

Hi Sorry to hear about your wife. I too have recently been having all the tests , some waiting for results. My protien/ ceratine was 399 and that petrified me. At present I've been put on Ramipril as its an Ace inhibitor and protects my kidneys, I'm very new to this and have my bloods checked again to see if the levels are down . I am following a CKD diet which is very difficult at first to get my head around . But tomorrow's Blood test ( 2 days wait for result) will let us know what happens next xx

Who considered 399 to be a frightening figure - primary care eg GP, or secondary eg, nephro?

For 3-4y, my mum's protein/creatinine has varied from 230 to 398 mg/g or 0.26-0.45g/24h. Her nephro is unconcerned at <0.5g and considers her proteinuria stable. Is yours?

He has not prescribed a drug because of that, because her BP is normal and because he is concerned about drug side effects. Conversely her UK doctor's surgery has been pushing ramipril which she has refused, siding with her nephro.

Note:

1. Mum's urine albumin/creatinine ratio increased 100%/y over the above period but we now think we may have stabilised it via Nutritional Therapy 🤞🤞No renal diet or dietician.

2. She has only 1 kidney which likely explains the relatively high albuminuria and proteinuria.

I'm not sure I have any answers worthy of a contribution, but I just wanted to say that your wife is very lucky to have such a kind hearted husband. I wish mine was that attentive.

Definitely something to get checked for with a Nephrologist. Is she diabetic?

eGFR of 64 at age 70 is really not that bad.

Yes she is diabetic - well controlled with Jardiance. The moderate eGFR coupled with the terrible protein-albumin numbers really confuses me.

Couldn't get in to see a nephrologist until late January... Life seems a bit on hold at this point.
