I know that Tylenol is often recommended for pain with CKD stage 3. But I am wondering if Tylenol PM is safe. I know that Tylenol PM includes an antihistamine to help with sleep. Does anyone have any experience with this OTC medication?

Any drug can have side effects. Tylenol works through the liver and is not harmful in moderation to the kidneys. An occasional use of it if fine. What is concerning is the other ingredients in Tylenol pm. This is truly something you should run by your doctor who knows your medications and history.
That med is generally okay for those with CKD - antihistamines and acetaminophen usually aren't harmful in limited amounts. But it's always best to check with your doctor. He knows your medical history. Simply place a call, he'll get back to you.
I agree with others about contacting your doctor. If your doctor takes time to call back and you’re in a lot of pain you could always ask any pharmacist at your local chemist. I’m assuming that the antihistamine in tylenol PM is one of the ones that make you sleepy. Is that why you want to take this formula? The antihistamine in it is not a pain killer. There is also a slightly higher dose tablet of the usual Tylenol dose that you can buy for pain but you’d need your doctor to approve
