My PTH level is 415, through the roof. I take a pill for it. CalcitrioL 0.5 mcg capsule. It doesn't help. I have very brittle bones, and I'm only 48.

Have you ever spoken to your relevant medic (or maybe a naturopathic practitioner) about natural treatments such as vitamin D? Ive shown 2 links below, for interest. Both state CKD benefits and:
(i) The 1st summarises, for CKD 3-4 "Best clinical practice supports vitamin D supplementation with either ergocalciferol or cholecalciferol for patients with CKD".
(ii) The 2nd includes in conclusions/recommendations "a few clinical trials have suggested that 25(OH)D supplementation lowers PTH levels in individuals with CKD stage 3".
I believe that medics are behind the curve with alternative treatments (little/no training) so Id expect most are negative. My mum (CKD 3) supplements vit D. Her PTH is ~25% through normal range and eGFR now up to 76ml/min although urinary protein issues remain.
I have high Pth as well. My nephrologist wants me to go on a vitamin D analog but I also have a condition called sarcoidosis which causes calcium to spike when taking vitamin D. If anyone has any other way to bring PTH down I’d love to know.
Of course, your kidney doctor should be taking care of this, but I can share my experience. Last year I had very high PTH on labs and low Vitamin D. My current nephrologist handled this by prescribing vitamin D (2000 iu) daily and calcitriol (.25 mcg) .with the calcitriol I started on 5x a week, then 4 x a week, and now 3x a week. Hes watching that the calcium doesn't get out of range. For several months now, my PTH has been down in normal range.

Hi Savreen1, I've had low Vit D levels around 8 - 12 since they were first checked in 2007. Naturally my PTH is skyhigh since it goes hand in hand. I was put on prescription Vit D 50,000 IU once per week for all these years. My Vit D level never reached over 16 at it's highest and that was only 2 times in all these years.
My Neph. was concerned and told me to quit the 50,000 IU prescription and he gave me another prescription for Vit D with Oyster Shell Calcium. Being allergic to shellfish and a vegetarian I couldn't take it but I found an alternative on Amazon with the same Vit D dose and seaweed Calcium. I've been taking that for a month and will be tested in 2 months. My Neph thinks that's the answer.
I also have to make sure to eat something when I take the Vit D or it doesn't get absorbed. Since I have IBS that also is a problem as most with IBS have low Vit D and high PTH. I was sent to an endocrinologist to try to manage my Vit D and PTH about 12 years ago. She diagnosed me with hyperparathyroidism. My Calcium is not high as is typical with low Vit D. The endocrinologist told me that the board of endocrinologist isn't sure why but something in the environment is causing a lot of low Vit D levels without elevated calcium levels.
Who knows, right? If the Drs. don't know how are we supposed to know. Are you taking a calcium supplement and eating with your vitamin D? A visit to an endocrinologist may help find some answers.

Anything over 20 on your blood test is within normal limits, but you are really low normal. My rheumatologist and endocrinologist told me they like to see my Vit. D level at least 30. I think they are dreaming but I keep trying. As long as you take the same amount of mg of Vit D it doesn't need to be a prescription as long as you get the same per week. It's just easier to take one high mg. per week than to take it daily. You can't buy the high once a week dose OTC you need a prescription. Plus it doesn't contain the calcium so you'd still need that.
Hopefully if you take the Vit D and weren't taking any before, your blood work will improve next time it's tested in a month. My rheumatologist told me that the lab tests should show improvement within 2 weeks of starting Vit D.
