My reticulocyte count has been at the high end of normal. My MPN doc said he would like it to be lower. Does anyone know what this means? I checked Dr Google but am still confused.

The KISS explanation is that the reticulocyte count test measures the number of immature red blood cells (reticulocytes) in your bone marrow. An elevation in reticulocytes can be a measure of bone marrow dysfunction in PV. This would be something that could be of concern. Suggest you ask your hematologist to better explain why the overproduction of immature RBCs is a concern and what it means in your case.
Trying to normalize bone marrow function is a goal in treating PV. I expect this is what you doctor is getting at. I believe you were weighing which cytoreductive agent you should be using. Trying to reduce your risks and prevent disease progression is the goal of treatment. Suggest you review how best to approach this with your care team.
