She said it may be that I'm still undermedicated on my.thyroid meds as body needs good level of T3. All so complicated

Have you been referred to Endocrinology? Having a thyroid that is not working properly can cause all sorts of symtoms in the body. It might take a specialist to sort it out for you but once your thyroid is sorted out you should see an improvement in general.
Yes I've seen endocrinologist. He said I am euthyroid now. But my T3 is still not optimal.
I get this too, but I'm not on Hydroxycarbomide for my polycythaemia, I only have venesections and I'm on a low dose asprin plus Lansoprozole.
I have noticed that anyone with an MPN can get these symptoms, and that in my opinion all treatments can cause iron deficiency, and this in return appears to incite a lot of pain and easily ache (painfully) from little exercise... I.e. hoovering or helping my wife carry shopping in from the car...
Have you noticed how it's always within 3 days? though for me it has been within 12 hours now...
