Severe Penis Skin Problem - HELP

Severe Penis Skin Issue for a few years now. Can someone tell me what kind of doctor I need to see? Do I need to see a urologist or Dermatologist? Also, does anyone here know of a doctor in the US in the DFW area that specializes in this? Any idea what this may be and any recommendations on something that will help?

First aid ointment or cream makes it substantially worse.....

I would suggest a dermatologist for the penis skin issue. Sorry I cannot help with recommendations for particular practitioners as I am in the UK.
