Hi All

Thanks all so much for posts it’s been a real help. Man I’m struggling big time with this. I’ve just seen a private urologist and he has said I have bxo. I’m just not convinced. I started out with rim of helmut being swollen and purple and it’s got worse . I have been given some steroid cream and he said that it could end up in circumcism which I would love if it fixed it but I’m just not convinced it’s bxo. Is also have dry scrotum and red which isn’t linked to bxo but he dismissed that. This is ruling my life and can’t see any light at end of tunnel. How long to steroids cream take to show any difference ? Has anyone here ever sought a second opinion? There is part of me that thinks I have to trust a urologist with 39 years experience but I’m convinced it’s not bxo.

Hi. I recently was also diagnosed with Lichen Sclerosus, formerly known as BXO. Like you, the symptoms were atypical - no white patches, no itching.

I saw a private urologist for a broken frenulum who suggested the underlying cause was LS. He referred me to a dermatologist who confirmed the LS diagnosis with accompanying psoriasis, which rather fits the description of your scrotum issue.

My suggestion would be to get a second opinion from a dermatologist with experience in penile skin issues. Your urologist may well be correct but the best way of exploring non-surgical solutions and treating all your genital skin issues would be by way of a dermatologist.

My treatment with clobetasol for the LS, and trimovate for the psoriasis, appears to have been successful and I am hoping for sign-off next month. An important part of my dermatologist’s regime has been to ensure no retention of urine in the foreskin, so always wiping or washing and drying, and no intact with soap, only a non soap emollient. I used the clobetasol under the foreskin and in the sulcus and frenulum, for 2 times a day for 5 weeks and then will have used the trimovate for a further 2 months by the time I have seen the dermatologist again.

For me personally, circumcision is a very deeply unattractive and disturbing proposition having never experienced any penis, foreskin or glans problems previously.

I wish you success but would strongly suggest seeing a dermatologist for a skin condition

I think I have something similar to you. My urologist was bit dismissive of the red scrotum issue as the shaft gets bit red too. I’m sure it’s a different issue like psoriasis but I don’t have it anywhere else . I’m gonna see what he says when I See him but I will mention those two treatments as they are different to the ones I’m on. The red scrotum issue just almost happened over night when I got out shower but after I had stopped an initial steroid cream timodone which made me think it was a reaction to that but now I think it’s something different like psoriasis

I also thought that my psoriasis was a reaction to an over the counter fungicide hydrocortisone but it was not apparently. I had never experienced psoriasis previously and it was only on shaft, mainly, and scrotum to a limited extent. The clobetasol is the initial LS treatment.

Never had any issue before with psoriasis. I’m more concerned that is bxo is on shaft and scrotum a circumcism isn’t gonna help it but the red scrotum and shaft looks more dry and scaly so I hoping it’s something different . So confusing how this has suddenly just come out . I’m convinced this all started with urine getting under the foreskin. Will see what he says in couple weeks

It’s very unlikely that LS is on the shaft or scrotum. Typically it affects the glans, foreskin and frenulum. Focus on non - surgical solutions now. Put surgical procedures to the side at them moment. Clobetasol works for 50-60% of guys over a 1-3 months period. My impression is that urologist are a bit dismissive of this approach as surgery is the traditional treatment - I note 39 years of experience comment - so get a second opinion from a dermatologist if needs be. Are you in the UK?

Yes in the Uk and have private healthcare . The consultant was full of experience and to be fair to him he was rushing out the surgical route so I think I need to give him another chance in the follow as I don’t want to undermine him at an early stage but before any surgical approach I would def get second opinion. A week or so of steroid has help the LS but the rim of head is still bit swollen it’s the random dry skin on scrotum and shaft that had thrown me . Thanks again . This forum has been useful

Hi Man it can be hard to get your head around it but give it time it will get easier. I think you should get a second opinion if anything it would reassure you. a dermatologist would probably be your best option. there are several different conditions with similar symptoms so it is difficult to diagnose properly really a biopsy is the only sure way to diagnose but most doctors don't do that they usually just diagnosed visually with there experience.

The steroid cream you have been given should help it will take time and it may be something you will need as an ongoing treatment as the BXO can return when you stop using it. So if it's not too advanced and you don't have discoloration around your meatus take your time it's not something you need to rush into get your second opinion and if it is BXO then way up your options whether you will need a circumcision or if you want to treat with steroid Creams.
