Hey man, I know that it can be scary, but that is Phimosis. It occurs when the foreskin cannot retract due to the phimotic band around the tip of your foreskin. Since you cannot pull back your foreskin, the head of your penis will be very sensitive hence why it may hurt if it makes contact with anything. I had a circumcision 10 days ago because of the same thing you're going through, and mine was quite severe. You have a few options though.
1. Stretch (slowly) the foreskin using a steroid cream, now mind you this will take patience, because if you stretch it too quickly you can crack the skin which will scar and make the problem worse. (The scarred skin will become tighter and less elastic, which is the opposite of what you're trying to achieve.)
2. You could get referred to a Urologist via your physician and take it from there, depending on how he addresses the severity, he may recommend that you get a circumcision or a dorsal slit. Both are quick prcedures but require time to heal. If he recommends that you can stretch it, there are plenty of creams and tools to use to help you along the way. The internet is a great resource for advice on this process as well.

I actually just had mine done 10 days ago. I had tried stretching so many times for months at a time but it just didn't help. Now that's not to say it won't help you. My phimosis was quite severe. The opening at my foreskin was the size of a pinhole. I couldn't urinate properly. So far, post-circumcision, its been fine. It's been life changing for me. It's healing fine and I had an excellent doc. The procedure is very quick and simple. Depending on what kind of cut you want you can make your penis look the way you desire. I highly recommend it if you have struggled with this for so long. (In less severe cases you can stretch it out and be fine, but that is your choice)

At your age the tightness you describe is not uncommon. Try gentle stretching whilst in the bath or shower. Your Doc' may prescribe steroid creams to aid stretching.
And yes, the head of the Penis can be very sensitive to touch. This will reduce over time with use and touching.
