I feel for you, know exactly what you're going through! Have you tried Hydroxychloroquine (Plaquenil)? i have Lupus, Sjogrens syndrome, APS & fibro .... I take Plaquenil & Methotrexate. I also have to take 210 mg of MST (tablets), Oramorph which I have to take a very high dose 30ml every four hrs! I also have Dihydrocodeine, Tramadol, gabapentin. Do you take Amitriptyline? I take them for muscle relaxant. I have to take venlafaxine (antidepressant) not because my illness but because of the 2 major strokes I had a few yrs ago! I hope some of this makes sense to you. I'm sorry if it seems a bit jumbled! I hope you can manage to get something sorted out for your pain! I know how awful the pain can be at times! Talk it over with your consultant or your GP & explain how bad it is for you, I'm sure they will be understanding! Good luck with it.
Consult with your doctor/s about optional Rx treatments.
Many Fibromyalgia patients are prescribed and have relieve with Rx Pregabalin / Lyrica or Rx Gabapentin - but these might not be appropriate Rxs for your individual situation.
I'm certain the Fibromyalgia effected members will chime in soon with their valued experience.

FYI : Tramadol does come in extended release form (called Tramadol ER) which for some patients works better than the non extended release form. Tramadol ER available in once a day dose of 100mg, 200mg, and 300mg. Rather remarkable stuff.

Thank you for replying I appreciate the advise. I've tried most of your suggestions but not as a combo - will talk to docs next appt. what I really need is something to take on a bad day, like you'd take a paracetamol for a headache! I am at a very high dose of oral morph 60mls per 4hours without much relief - I've become so tolerant over the past 15 years. I went to a pain clinic which was a waste of time. They were more interested in lowering my dose of opioids (which I understand) rather than helping with pain relief.
I do have a fentanyl patch (more opioid) and used to have fentanyl lollipops which were great but it cost the nhs so much they, apparently, are only for cancer patients now.

Hi I try to stick to Naproxen & Tramadol as they allow me to still come to work and my brain to work as clearly as possible. I have MCTD & Fibro and have sometimes had to resort to butrans patches. The downside is they make me a bit of a space cadet so driving is a no no...
Hi, I was recommended to use Bu-Trans Patches by the pain clinic. I started on 5mg and am now on 10 mg. you put it on the top of your arm and wear for 7 days changing at the same time every week. As it releases a constant source of morphine in a low dose it is used alongside tramadol and paracetemol. Ask your pain clinic or reumy.
