A national survey is being carried out in Wales to hear from people with lupus about their experiences of care in NHS Wales. Your feedback will help us understand how these services could be improved.

This survey will close at midnight on Monday 15th April and should take around 15 minutes to complete. Your response will be anonymous unless you choose otherwise.
