I am trying to apply for PIP, but i called the customer service and asked for the application. They asked me to get DS1500 from my gp. My gp receptionist said the DS1500 is for people with terminal disease that have 6months to live. This my first disability application, pls can someone explain what i should do.

If you can't find the information you need on the website find a local CAB and ask their advice. The DS1500 is a special form for the terminally ill so that their claim is processed faster - it can take up to 4 months normally - so I assume the person you spoke to misheard what you said to them or doesn't know their job...
Don't try and complete the forms on your own. Get advice and support from the CAB, or one of the very good online support groups. I use Benefits and Work, they do change a fee to access the site but the information is really good and explains exactly what to put in each box
You need to go to benefitsandwork website and they do charge a fee, but it will be the best £20 you'll ever spend as they show you how to fill in the forms, and also how to appeal if you get refused.
It's a very valuable website for anyone on benefits.
