I have my appointment in 2 days- I have written a 2 week diary of how I feel, what meds I'm on and my medical history.

Do I need anything else?

I don't have any other conditions that I know of and no doctors are treating me at the moment which is why I have been referred. At the moment I'm on warfarin and clexain what out of range, nothing more nothing less.

Should they not have all my details on file?

Yes to the things you have written down so far, plus the addition of info as suggested by APsnotFAB - any remember any detail. as sometimes what can be thought of as minor detail in a situation could be a clue! Mary F, best of luck with this. x
