INR Crazy times!

Go figure! Last week at 4.7 little higher than I need so as I self medicate and adjust dropped from 8mg 7mg alternate nights to 7mg for 7 days and arrive at 1.8 tonight!! No diet changes no increase in anything so so frustrating. The only change is a change in pain med from 15/500 to 30/500 codeine but can that cause this?

Dont know about codeine but have you got dome heparin while your INR is so low?

No just going to hit the warfarin button large for a couple of days usually does the trick!

Too many nights in a row of 7 mg.

Some hematologists like to see what a weekly average of mg total is, as well as day to day, what will hit through in 48 hours.

When you look accumulated 7 mg for 7 knights in a row- that totals too much mg loss.

I’ve also learned to do 1/2 and 1/4 doses.

You might try 7.25

Are you bridging with clexane for a few days until it comes back into range?

Sorry Greenmil- I’m just seeing that you said you you did alternate the nights that you lowered the mg, not 7 nights in a row.

It still must have been too much of an accumulated drop in mg.

( sorry, I’m in a migraine today, I apologize. I had a rituximab infusion a week ago today, and an infusion lowers my antibodies- so I have noticed I have to lower my mg on warfarin also. It’s the warfarin roulette for me also right now. I had to inject over lunch. I find it very helpful! )

However, with my already poor vision, add a low inr and my ability to read is diminished. Sentence quiver and warble.

I find that, when I hit 4.7, then I just take 2 or 3 for one night (or 5’s for 2 nights) and I’m back in range (3.5). If I’ve ever needed to stop warfarin for surgery, I’ve usually dropped down to 1 within 2 days so mine does drop pretty quickly but yours may take a little longer. My INR is always affected if I change any of my other meds or I’m in pain.
