Is myofascial pain and TMD a part of Fibromyalgia or separate conditions?

They are both conditions in their own right that are also symptoms of Fibro. Just like IBS and Migraine are both symptoms of Fibro but also conditions in their own right.

All of these conditions require targeted treatments - what will work best on the central Fibro may well not be the best treatment to deal with these issues.
