Go on to Benefits and Work website and fill in their form, which will make it easier for you to remember things which you might otherwise forget and leave off. You can also get in touch with CAB (although they have reportedly sold out to DWP, in order to get a grant from the government)

Do NOT overestimate what you can do, and DO NOT underestimate what you can't. Before you start the form, make sure you have made notes of when you couldn't complete something/someone had to help you to complete something safely/and in a timely fashion (If it takes you more than double the time an able-bodied person takes, it means you can't do it).

I have been in the support group for many years and had my first assessment March of this year. As Ajay said use the words long-term, progressive and severe. If possible never give a yes or no answer. On my form I think I put it varies to every question. List all your medications and how they affect you. Also list all medications that did not work. Do not rush the questions. Do one or two questions a day. Think who helps you do what. Remember all tasks must be able to be done repeatedly and safely. Write about your day for example I get up take my painkillers then wait an hour for them to work. I then wash dress etc and take all the things I need for the day with me so I only need to go up and down the stairs once. I also note that I have double hand rails for safety. Also I cannot go up or down stairs repeatedly. I also noted that on occasion I sleep downstairs as I cannot climb up them when I am bad. The more detail the better. I have been told not to do things repeatedly, to pace my day. Yes I probably could walk 50m twice with a rest inbetween. However after that I would not be able to do anything do to pain. Obviously if a consultant or specialist as given advice about what to do or not to do include that. On my f2f I only completed or tried 3 tests on the upper body and none on the lower body as they caused too much pain.
