Esa113 form filled in by a doctor unknown to me ?

Hello hope everyone's as well as can be today, I'm just wondering if anyone has heard of this esa113 form , it's basically a form esa department send to gps to ask them questions regarding the esa50 form we have to fill in, only I've come to view of mine and the gp who filled it in doesn't know me and she has missed a lot of information and has ticked a box stating I can travel to their office by taxi or public transport yet my own gp has stated otherwise in my supported letter this gp knows me what I don't understand is why a gp I've never seen can fill this form in with missing information and wrongful information no wonder they send me an appointment at their office. It's all contradictory 1 gp says this an another go says that, the gp who filled my form in clearly did not view my notes, an I'm fearing now that this can go against me at my assessment talk about raising my anxiety 😩

Thank you Ajay575 I see maybe if my actual gp had filled this form in then maybe I wouldn't of got this f2f but at least they accepted my f2f to be done at my home even after this form was sent to them, no wonder the Sep are confused about us claimants it all looks contradictory I just hope it doesn't against me at my home f2f . Thank you I wouldn't like to question it but my mum thinks I need to contact this gp and ask her to print off my notes on file as it seems she's not looked very well even to just get some background on me seein as she doesn't know me but I don't like to question gps either but I wouldn't want it to go against me and I lose my f2f so it's it's a hard decision.... thank you again 🙂

You can ask the GP who knows you for a summary of you health conditions and how they affect you. Your mum is right. You should contact the surgery and let them know that some of the information the other GP has give has not been in full and as a result has been misleading to the reader.

Yes I think this is what mum means asking for my notes, it is on file about my referral for a wheelchair etc I would assumed this other doc would even assume my walking abilities are limited just seeing this on file but seems not so looks like a phone call to this doc is needed, thank you for your advice bobbybobb 🙂

Hi Ajay575 yes I can see all my conditions and diagnosises on my file etc but this doc failed to put these down on the form, although she has put my mental health diagnosises on this form but missed out some of the medication for these even though I always send in my prescriptions anyway to the esa with my esa50 it just thrown me seeing this form with missing information on it hopefully the assessor will see my difficulties when they assess me again. That reminds me I got my wheelchair last year or year an half cant quite remember but I didn't send any evidence of this in I wonder if this would make a difference? Although I have sent in evidence of my wheeled seated walker and all my other aids and adaptions and this doesn't seem to of made a difference so maybe the wheelchair evidence won't make a difference either haha🙂

Yes I believe this is true, although I don't use a wheelchair that is self pushing I have one that has to be pushed as I can't use my arms to wheel myself my arms are to weak with this fibromyalgia an the fatigue and pain etc i was given one to get me out further than the wheeled walker but I hardly even leave my bungalow anyway coz of anxiety an the pain and exhaustion it's a viscous cycle I do get pup standard care enhanced mobility maybe this will go towards my esa this is my second esa assessment I will be having my 3rd pip assessment this year too I wish they would understand this I isn't getting better but I have to go along with their rules and have these assessments every year it's soooo stressful I was denied flare 8 years or so ago for all this but I passed the pip 2 years ago but I know people on pip can also work so I understand the esa and pip are different well I thought so anyway.... thank you Ajay575 🙂

The Practice Manager is there to help with matters like this where one GP does not cover the case correctly or submits information that is contradictory or inconsistent with what another GP has said. Ask to see the Practice Manager, explain the problem and ask for them to sort it out and for the Practice to write again to DWP, explaining their mistakes. Ask for a copy of the letter to be sent to you as well.

Hi there.

Ring your GP and ask for a full medical history, don't be frighten of questioning your gp.

Maybe when your practice got the form your gp wasn't there?

I don't know how you got yo read what your gp wrote I have never heard of that.

Just explain to the health Pearson that comes to do your f2f that you have never meet the doctor.

The thing is unless it says on your file home visits only that GP would think that you went to your doctors, so there fore he would be right putting in that you can get to your practice.

I manage to go to my pratice all the time but feint mean I am fit, so don't worry about that sort of stuff it's irrelevant.

I would have my f2 f recorded I did. You have to tell them you want it recorded.

Listen to your mum, we are not always right but in this case she is.

You might have to pay.

Sorry I forgot to say there alway at this my husband has just got a new ESA form to fill in, he's worked most his life never been on dole never ever claimed benifits and not long to retirement and there at it again.

I myself was at it before Christmas again but I rung them up and said I wasn't doing it that I had had enough and to do what they wanted,🤦‍♀️

Lucky for me the man asked was there anything they should be aware of as I was crying on the phone and said I couldn't cope so I told him straight yes there us I gave cancer just finished chemotherapy and having radiation therapy.

He was lovely, makes a change he just said ok we will contact your GP to verify this and not to worry.

I haven't heard anything yet!

But wouldn't surprise me next year they send me another, I am a perason who has be disabled through back problems from the age of 18 but worked all my life till I was 57 then fibromyalgia got the better of me I would have been getting my pension 3 years ago if they hadn't put the age up.

Roll on 66 I say then they can go and swivel.

Your GP can only say how he thinks your illnesses affects your daily living. As he is not with you 24/7 that’s the bast any GS, Doctor can write a snapshot and that’s all the acessment process does it up to us to submit the evidence and examples of how our conditions affect our daily lives,

Hi,

Just to say you can now get a full copy of your GP file by request. Possibly you could use extracts from this to add to any dip application?

Just a thought.

Myself I had to change from a go that knew all my medical history, problems and effects on my day to day life to a go (different practice/area).

The new gp has caused me nothing but stress and anxiety as never bothered to read my file, knows nothing about me, appears to dismiss cfs/me and fb as a physiological disorder as I was tearful when I see him. Because I feel so dreadful. He is unsupportable, dismissive and patronising to me. Refused my regular bloodsp tests (always done as a safeguard also with other medical problems). Basically I avoid as upsets me so much and it’s not uncommon for me not to be able to get to my appointment on the day, to which surgery doesn’t like.

If this go did a form for me I’m certain would do the opposite to supporting me.

So I do hope you get sorted with your go. At very least I feel we deserve our gp’s support in all ways.

I'm going through this now. My current G.P is new. I've seen her twice and spoken toh er 4 times on the phone. My husband always takes me to the surgery in my wheelchair-but as the actual Dr's office is small I always get out and walk in so she's never seen me in it. When I filled out the ESA form I also sent the Dr a letter and a short form I downloaded as I'm a member of Benefits and Works which highlights my conditions, how they affect my daily life and why I cant work. I acknowledged that its only my say so but at least its on record at the surgery. I dont know if it will make any difference
