I too, have finally found the courage to apply for PIP after being advised to by many people, including the lady at the Job Centre who said I was definitely not fit for work.
She could not understand how on earth I "passed" my ATOS medical.
Please do post how you get on.
I phoned and requested the forms last week. I'm still waiting for them to arrive.

I had my PIP assessment today, It took place at my home this morning, The Atos health person was a nurse and was very kind and understanding and had taken time to listen to my everyday difficultys..... I was very tired afterwards the interview took 2 hrs 5 min's.
Just glad its all over for now, Told that I would hear something from DWP in about 6-8 weeks time
it sound as though you have had a good assesment nurse who took time to listen to you hope it all is taken into account and you get a good result.gentle hugs
