Bad first stim scan

Hi, just on my second cycle of IVF. First cycle was cancelled as I didn't respond well to the long protocol. Only had one follicle in left ovary and even that was too small. Right ovary didn't respond at all. On my second cycle, short protocol and just had my first scan this morning and nurse couldn't find either ovary. I'm so disappointed.I have another scan booked in for tomorrow morning but not sure what will change in a day. I thought this part would be ok as before starting IVF I was ovulating every month and my scans showed that everything was working well and nothing wrong on my side except low amh. My partners sperm count is extremely low so I thought the difficult bit would be getting a viable egg to implant.

Should I just give up.

Hi hun I’m so sorry you’re going through this. Do you know what your AMH and AFC are and what meds/dosing you’ve used? What day of the cycle are you now? Maybe the stim dose just needs increasing a bit for you. Some women benefit from priming too. Crossing fingers all not lost for this cycle xx

Hi, my amh was 4.89 and my afc at the time of scan with doctor originally was 7 and one egg as I was near ovulating at time. I am on 350 of pergoveris and 150 of gonal f also started cetrotide 2 days after starting above. I was on estrogen patches before the cycle also.

Don’t give up lovely. It can take time to find right protocol. Long protocol was probably much too suppressive with your AMH. I have a similar AMH. I didn’t find oestrogen priming helpful (it slowed my follicle growth right down and still didn’t even them all out). Have you tried a natural start? Also cetrotide seems very early to be started day 2 of stims. I’d speak to them about that as usually it’s day 5/6 of stim injections that the antagonist is started in a fixed start protocol. Starting it too early can also impact your response and follicle recruitment. I found Pergoveris didn’t suit me as much as just using 450 iu recombinant FSH (everyone is different tho). There is some theory that FSH only regimens better for us with low AMH. Hope you find a protocol that does the trick. Don’t give up xx

Thank you. It's good to hear from someone who has a similar journey. I feel like my clinic aren't really tailoring my protocol to my specific needs. Feels like a copy and paste treatment plan so maybe that's why they haven't taken into account that suppressing so early might not suit me.Haven't had a natural start, first cycle I was on the pill for a few weeks and then the injection that puts you into a menopause state before starting stims, and this cycle I was on the patches beforehand.

I should say I have a child by natural conception when I was younger.

I might try changing my clinic xx

How you respond to treatment protocols can be quite different with each cycle Might be an idea to discuss your concerns with your consultant/nurse

hey. I had a very similar thing to you. I have an AMH of 1 (very low for my age 36) and my AFC was about 7/8. My 1st round high dose of drugs after being on norethistorne. I only had 2 follicles respond and my left ovary did nothing at all. They did the stims for a bit longer and I only had 1 follicle big enough at egg collection, sadly no egg.

My 2nd round different drugs and a lower does. Still put me on norethistorne first. Again only 2/3 dominate follicles at first scan and this time my right ovary wasn’t doing anything at all. Second scan I had 2 big enough to collect so didn’t need to do extra days of stims. They did the egg collection and got 2 eggs (1 damaged and 1 good). Our 1 egg fertilised but sadly was a negative after tww.

I found them changing the drugs helped with getting bigger follicles. But again I had a lazy ovary both times just not doing anything. I have read things about norethistorne shutting everything down and I worry this is why they don’t both respond. The consultant said it wouldn’t do that but who knows.

Our 1st round was nhs and I wish now we had cancelled it and not wasted it going to egg collection and just tried different drugs first. It’s so difficult as you listen to the experts. Plus you’re just desperate for that family you sometimes do your thinking after. I’m not sure if this is your case or if you’re paying.

Don’t give up. As everyone says only takes one egg! Cliche I know and doesn’t feel like it when everything feels like it’s going in the other direction. Xx

Hi, I was over the age limit here for free IVF so am in the lovely pricey private sector. But I am following what the experts say as you said and trusting they know what to do ,plus I hadn't a clue really when I started just thought I'll follow what they say as they know best but at this point I think they haven't actually tailored the protocol to my specific needs so if I have to try again I'll research on the clinic itself.Everyone here is so nice I wish I had found this sooner.

Had another scan today but with sonographer instead of the nurse and she found 4 follicles in my left ovary, two were size 10, one was 8 and one was 6. Not sure if sizes are good for now in cycle but hopefully they progress. Couldn't find right ovary still. I have another scan on Friday with the consultant so hopefully that goes better by then. Then he will decide what the plan moving forward is.

They are experts and well trained in what they do. I just now strongly believe going in with your own knowledge and ability to question what they’re doing is so important (learnt from experience now).

We are going abroad for our next round as from what I’ve looked in to and spoken to people about it’s a lot more individualised and cheaper 😬 The forums are amazing, someone described it to me as the worst club to be in but the members are the best.

I can’t really remember the size of mine at scans now but I know the dominate ones got to the size they want to see. Fingers crossed for your next scan them growing some more. Each time I went for a scan they had grown (except for the ones which weren’t responding at all).

You could ask your clinic if they duo stims? It seems pretty standard abroad for those with low ovarian reserve. I think it may be suggested to us next month at our consultation with the Spanish clinic.

Had my scan with the consultant yesterday. Still the 4 follicles but he said only 2 maybe 3 would be viable and with my partners sperm count being so low I'd only have 5% chance of getting pregnant so there was no point continuing in his opinion.He also said no point in doing another cycle as it would be the same outcome. Kept calling me old and that my eggs weren't good enough. He told me the only chance of getting pregnant would be egg donor and use my partners sperm as my womb lining is good and the chances of getting pregnant that way are 50%.

Feeling so down and defeated and now like I'm an old woman who shouldn't be doing this.
