hi all, we are starting a new fet with a pgta tested in January. I’m still deciding between the natural method (I have regular cycles) vs medicated- we had two failed medicated fet in a previous cycle.
I’m a little confused about how natural modified FET works or is timed ie ovulation daily testing, where scans would come in, trigger and then the actual day of embryo transfer. Just wondering if someone could shed some light on their natural modified FET timeline to give me an idea? Thanks so much!

Each clinic have their own natural modified protocols. My clinic abroad wanted one scan on day 2 of period to ensure all was OK to go ahead this cycle. Then they wanted scans from day 5-6 (I ovulate early day 10 usually) every 2 days or so to identify lead follicle and once the follicle is over a certain size (can't remember what is this) you administer the trigger and then they time the transfer. In my case it worked with completely natural. I had a scan on day2 and then I started testing from day 5 with ovulation strips. Instead of several scans. I flew there on day 8 and on day 9 had a scan which showed a big follicle and lh blood test which confirmed lh rise and lining thickness. They said I wouldn't need the trigger. I had another scan on day 10 which confirmed that I ovulated and then they booked my transfer. I only had progesterone supplementation.
Personally, if there is no clinical reason for the doctor to recommend one or the other, I would choose natural or modified natural over medicated. It avoids the estrogen tablets and the downregulation. Although it involves more scans.
The best idea would be to ask your clinic about the time line as it can vary clinic to clinic Treatment is carried out using your own menstrual cycle and uses low amounts of fertility medication Remember if unsure of what is happening just ask for it to be explained to you again Take it a step at a time
I've had a lot 4 frozen transfers so far. The first 2 were medicated and both ended as chemicals.Then we decided to go for a modified natural fet. Firstly, It was so much easier on the body. From day 1 to day 10 you do absolutely nothing. Around day 10 you have a scan to check lining and lead follicle. If all going well, from there you will start ovulation testing, you may have another scan a few days later to check progress again. When you get your surge on the ovulation tests, you let them know and usually about 2 days later you start the progesterone. Then approx 5 days after that is transfer.
My first natural was successful but I miscarried at 9 weeks.
Following that the clinic wanted to try the same protocol again as we had gotten furthest with that. We did exactly the same again, but as it looked like I would surge on a weekend they also gave me a trigger shot the second time. That transfer ended in a negative - although beware testing early if you have the trigger shot as stays in your system for around 2 weeks.
We had then rub out of embryos so did another egg collection. We had not previously done PGTA but did this time and have two precious normal ones.
We just started a third natural fet, however I did not surge when expected and the cycle was cancelled on day 18. Which I am ok with as I do not want to waste a tested embryo, but just to make you aware it could happen if your cycle is even slightly off that round. We will be starting again in January with a fully medicated cycle.
I had one failed medical FET, included letrozol and ovulation trigger (unsure why I even needed medication, according to my clinic I have a lot of eggs and it would help, spoiler alert… it didn’t). I have always had regular periods and struggling with male infertility. After that failed FET I demanded a natural FET, witch meant I had to track my ovulation from like CD 9 ( I usually ovulate between CD13-16) and go in for scans once or twice. Today I’m 6 weeks pregnant because I trusted my gut feeling.
Personally, when I had my natural cycle I felt so much better, no headaches and much better sleep.
Unfortunately, there is no guarantee with transfer, so do what feels good for you. Best of luck 🫶🏼🌸.
Hi Icykoala88, I had two natural modified cycles at the same clinic and the timelines were different, so hard to say the protocol as it is much more fluid than a medicated FET. On one, I had a scan on day 2 and then ovulation tests from day 8. On the second, I had nothing until a scan on day 10 and then they triggered ovulation on day 11 and timed my transfer. The only drugs I took were the ovulation trigger (round two) and cyclogest from 5 days before transfer. The natural cycles feel much kinder to your body and mind but my first was cancelled pre-transfer as they weren't 100% sure of my timings and I felt like my period was coming, so we cancelled just before transfer, so it is much more unpredictable. My experience was - I did 4 natural modified FETs as I had a regular cycle . All negative
Went to do another natural modified cycle and everything looked good. One lead follicle on day 12 but my specialist wanted to check my estrogen just to be sure
It wasn’t rising correctly so we cancelled it as it was my last embryo.
We switched to a medicated cycle and it worked so for me I’m glad he looked into my hormone levels a bit closer and found out I needed the extra support.
