Experiences with mild IVF at Create London anyone please? Or any PM London recommendations please!40, LOW AMH, HIGH FSH plus male factor.

I don't know where to start. I am running out of time fast. Negative test result today.

Experiences with mild IVF at Create London anyone please?

I am considering this as my next move. Or would you save yourself the hassle and consider donor eggs instead? Two rounds, I have got only one embryo which has failed. Im not 100 sure i can get my head around donor even though my sister is willing.

I am 40 in three months, AMH probably around 1.8 or less FSH above 9 or more.

(figures a couple of months old) My folicle count is ok (10 ISH) but on my last round even though 6 or more responded I only got 4 eggs.

Background

August ’23 - failed ICI round

300 Meriofert 

Fymadel 

Three eggs collected - Two eggs fertilised non made it past 3/4 cells, nothing to transfer

Feb ’24 failed ICI round

Meriofert 75

Fostimon 225

Fymadel 

Gonassi 5,000 + Supercur ( Supercur WAS not available at the time so was substituted for Decapeptyl)

On the night of trigger Calcuim Ionohore was offered ( we used this)

Four eggs collected- one egg disintegrated on injection. Two did nothing. One egg transferred on day 5 as an early ungraded blastocyst.

My husband is not as keen to carry on privately this as me. I have had trouble getting him to stop drinking at weekends the past few months. On our failed test he said he is not sure we should go for a round private saying ‘last year has been horrible’ ‘its a massive gamble and the chances are so slim’ he said ‘’we know my sperm motility is still not great and that won’t be improving’’ Implying that’s the best you are going to get and he’s not trying to improve more.

So sorry to see your BFN :( It utterly sucks.

Maybe take a step back and breathe for a few moments - I totally get that your keen to forge ahead but try not to make any rash decisions.

40 is not too old! I would say the majority of us on here are 38+ so don't count yourself out.

I've not experienced donor eggs but so many people on here have had really positive experiences - and ultimately success. I imagine it's a lot to process but it might be the way forward rather than putting yourself through another egg collection that might not yield good results? Also, this is purely personal but I'm not sure I would go with a donated egg from my sister - it would feel like too close a link to me! But it absolutely depends on your relationship - I think I'd feel envious that my sister was the biological mother. Not to say that it's not the most generous and selfless thing to offer.

Could you explain to your husband that not drinking for the next 3 months is really nothing in the grand scheme of things - get him on proxeed or something similar to help boost that sperm. I really took control of my husband's diet through IVF by just making loads of healthy food for us both - he had no choice - haha!

Perhaps talk to him about why he feels so negative about it - it may be something deeper?

Also, have a friend who had success with Create - she had very low AMH and they did only get one blast per cycle but eventually one stuck.

Sorry to hear about your failed transfer 😔

I've done 3 cycles of back-to-back mild IVF with Create to freeze embryos (low AMH) and 3 FETs - I'm currently pregnant from the third transfer. I haven't experienced any other protocols or clinics.

Sorry to hear about your negative test. I know that it's not the news you wanted. I remember one of the nurses saying to me after I got my first negative (first round was free, the rest we self-funded)...she said that IVF fails more than it actually works. I've read it before buts it's true...it's a numbers game and it only takes one good egg. You are still young, please do not give up on your own eggs just yet.

JoyfulStar found success with create London at the age of 43/44.

Hi your AMH is not that low and your follicle count is good so don't stress and your age is still fine. Just so you know egg quality will decrease during high stimulation which will ensure the eggs created using natural IVF will be better (this will be my next plan too).In your case your husbands motility wouldn't make it better. Have you checked morphology and DNA fragmentation? This will most likely contribute to the quality... If he drinks a lot, more than 5 units a week it could directly be contributing too it. I guess the best thing (that will hopefully give you the highest chance) would be you and your husband taking 2-3 months out to relax, eat healthy, reduce drinking, take supplements to improve motility / egg quality and then try a few rounds of natural IVF at Create. I attended one of their presentations and they seem good.

Maybe read 'It Starts with the Egg' for guidance on sperm and egg quality.

Have you any experience with dna fragmentation testing - I’m not sure about what they can actually do with the results. I had a feeling about this with the eggs - but as I’ve never had anything to freeze so far i Wonder if Embryo banking would be useful for me

He is on l’carnetine and r alpha lipoid as per it starts with the egg - that’s the only reason it is where it is now. I’m not 100 sure of his latest stats as the clinic hasn’t been direct in sharing them with me - I need to get them.

Also I’m really not sure of my up to date amh and fsh as I understand this can go wild after a cycle ?

I also watched the virtual meeting but husband came home half way through and wasn’t possible to continue watching

No I haven't had any personal experience with DNA fragmentation. My partner had low motility and low morphology and managed to improve that by stopping one medication (bad for sperm) and taking supplements. So we never did this test. Is he taking multivitamins too and omega 3?L-carnitine and ALA is good but he needs multivitamin too with zink and folate/folic acid. The results would give you, your husband and the clinics more information. It could be the reasons the embryos failed too.

Yes FSH can fluctuate a lot.

Which multivitamin do you use ? I had him on wellman but it’s not high in all that much good stuff and expensive . Oh Coq10 as well x

Vitabiotics do a men’s conception vitamin. My husband is on the max version and I'm on the equivalent for women.
