Looking for some advice pre transfer

I was just wondering if any of you lovely ladies might be able to offer any advice on our current situation.

I am due to have a transfer on Wednesday with a euploid PGTA tested embryo (currently frozen from a previous round) following a fresh IVF cycle which sadly produced no embryos.

Our clinic have said that my lining was good (triple layer) at my last scan on Wednesday (EC was yesterday) so from their perspective we are fine to go ahead with a transfer this Wednesday with no need for any further scans/tests/appointments before then.

However, because this is our only remaining PGTA tested embryo, I would really like to be re-assured that my body is in optimal condition to accept this embryo after EC yesterday as we are kind of pinning all our hopes on this one taking!

Is the quality of the lining the main factor here and am I just being a little paranoid or are there other tests that can be done between EC and transfer day?

Also wondering whether it might be better to wait and do a frozen transfer at a later date once things have calmed down after EC, but because I have never responded well to stimulation drugs I was hoping this wouldn’t affect things.

Apologies for such a long post! Any advice would be greatly appreciated 🙏

I am by no means an expert but I personally would crack on and transfer if your lining is looking good and you have a top quality embryo. If you freeze it you risk it not thawing etc. heaps of luck xx

Thanks so much for taking the time to reply. I probably didn’t explain myself very well above but it’s not a fresh embryo as it’s from a previous cycle. We sent it away for PGTA testing hence why it wasn’t transferred fresh. It’s all a bit confusing because it seems to be still classed as a fresh transfer as it is following an IVF cycle 🤷‍♀️

I also just wanted to add that unfortunately we didn’t get any embryos from this cycle as the one egg that was collected didn’t make it 😔 so we don’t have the option of a fresh transfer x

Honestly I prefer fresh transfers over frozen because they worked for me twice and frozen transfers didn't... It's just a preference I guess

Oh I understand, really sorry for not getting any embryos this cycle

The reason I prefer fresh is that I feel that with frozen I struggle to get the lining to great thickness but with fresh my lining is always great and ready

In your case your lining is in great condition since you started as a fresh cycle, crossing my fingers for you!!

Ah ok that makes sense, I have never had a frozen transfer before so that’s a really good point about the lining. Thank you so much for the advice, it’s much appreciated! xx

Your welcome!Honeslty I had 6 transfers and I really don't know how some embryos implant and some does not even though they are top quality

Hello hun, I wanted to offer a bit of a different take, which I hope doesn't just confuse things further. My understanding is that exact timing if transfer is one of the key factors for implantation, as well as of course embryo quality (which is the biggest, and you're covered on that!). I had the ERA test done to check/determine the exact right timing and transfer protocol, before my successful FET. Since you have one beautiful perfect embryo frozen and safely waiting, if you can afford it financially it might be worth doing the ERA test first, before then later doing a frozen embryo transfer.

If it helps at all at my clinic they actually have better success rates with FETs than with fresh transfers, and for me it was a FET that I had success with. I did a natural modified FET cycle (using my natural cycle but just adding an ovitrelle trigger so we could time things exactly and then progesterone support). Make sure your clinic give you plenty of progesterone support and if you can get a blood test on the day of transfer to double check levels are good.

This might not apply to you at all (and I don't want to scare you as it's only a minority of us that struggle with this), but if you think you could have any immune type issues it might be worth asking your Dr if they would consider prescribing a low dose steroid for your transfer (for me because I have elevated natural killer cells steroids are crucial to prevent my body rejecting the embryo, but as I say that's only the case for a minority of us).

I hope this isn't too much information, I don't want to come off as scare mongering, I'm sure if you decide to go ahead as planned on this cycle you'll also have a really excellent chance, as the embryo being genetically normal is by far the most important factor and yours is beautiful. Hi IVFat40 thank you so much for taking the time to reply. I really appreciate all of your honest advice and I don’t see it as scare mongoring at all! I wasn’t aware of any of those tests so I will definitely be looking into everything that you have mentioned before proceeding, that’s really useful and good to know so thank you!

I am currently taking utrogestan pessaries (3 a day) but I’m really keen to get my levels tested now if that’s something they can do.

Im not sure if it’s worth mentioning that I have had two previous fresh transfers, one of which was a BFP but sadly miscarried at 8 weeks, I just wasn’t sure if this has any impact on the immune test that you have mentioned but probably not I’m guessing?

Oh I'm so sorry to hear about your loss at 8 weeks hun, how devastating. The fact that you were able to get pregnant does suggest though that you don't have issues with implantation/early loss (which was my big issue), so as heartbreaking as I'm sure that was it may be a really good sign for your future transfers. And given that you've had previous success with fresh type transfers that suggests they work well for you too. As a side note if for some reason your clinic don't want to test your progesterone levels you can get that done privately online (through Medichecks), but hopefully your clinic will be helpful. Good luck hun, it really does sound like you've got every chance of success this time.

I’m not an expert but a progesterone test to ensure your levels are high enough could be helpful? I had two transfers just with pessaries and no progesterone test, on my third transfer I had pessaries and Lubion injections and the progesterone levels were on the lower end of acceptable (so I presume they were too low for the first two transfers).
