Low AMH and IUI or IVF?

Hi everyone.

I have had my AMH and AFC tested. My AMH is 3.9 pml I and AFC- 5 follicles( is it too low ) . My doctor kept on saying that naturally would be better for me and it's possible to get pregnant naturally as my uterine cavity, lining and shape are perfect for pregnancy. He said why I want to do IVF if I have a chance getting pregnant naturally. We are booking new semen analysis for my husband to see his results. He had abnormal head shapes in 2020 when he got tested.

My husband are on supplements now so hopefully his semen improved.

My question is would IUI be suitable for us if my husband semen analysis will be ok? Anyone experienced same thing?

Is your husband doing DNA fragmentation test as well? In order for IUI or naturally to work, the sperm has to be in a good shape. I have extremely low AMH, so IVF was useless for me because no matter what stims I had (high, low, none) - I only ever kept producing 1 egg. After year and half of ttc, I finally got it done naturally. I'm 37 weeks at the age of 42.

Congratulations Hun. Such a positive news. 😍. We booked privately to check hubs swimmers again and after we test DNA fragmentation too. And then we will decide what to do. Meantime we will keep trying xx

Hi Madeline

Marisa is right. Before you consider IUI you should have your husband do a sperm DNA fragmentation test to make sure his sperm aren’t damaged. It’s possible for his semen analysis to come back normal but for there still to be DNA damage, so you really need to do both tests to be sure.

If both tests come back normal, then IUI might be something to consider. It has success rates that are significantly lower than IVF, but it’s also a lot less expensive and less invasive.

Hi Madeline, can I ask for the reason you want to do ivf? If it was only because of your lower AMH and assuming you are 35 yo or younger, I would not worry and try naturally. Low AMH have about the same chance of getting pregnant as normal AMH levels, in both women one egg gets released each month. But if you are older, have other fertility conditions (such as male factor) or want more kids in the future, you might want to consider ivf. I have lower AMH than you and got pregnant naturally twice in a short span of time. Miscarried both pregnancies due to genetic abnormalities, which cost me a year of my precious time with AMH getting lower in the meantime and egg quality further declining. I decided to do ivf with pgs testing to be sure only viable embrios are put back in to minimise the chance of further miscarriages. This also allowed us to store couple of embryos for the future because with AMH and egg quality further declining, the chance I could do the process successfully again in 2 years time is low. I hope this petspective helps. Final decision depends on your other conditions and personal circumstances. I agree with doing DNA fragmentation for your partner to know your chances for a heathy pregnancy. Baby dust to you whatever you decide to do.

Thank you hun.. I will be 36 in January. My doctor told me same thing as you. He said that I am young and he is not worries about my egg quality. He really wants me to do naturally. We are waiting our results now and we will def DNA fragmentation for IUI too. Xx

Hi Madeline I think you need to see what the semen analysis is before making up your mind. To do IUI you need lots of pointy good swimmers. Bad shaped sperm cannot get into the egg, so at worst you may need to have IVF/ICSI. Of course there is the natural option, but be prepaired for a long wait. Good luck! Diane
