Hi there, it sounds like you should be using an ovulation predictor kit. Have you already got those? You pee on them like on a pregnancy test. You can get them at the chemist, and your wife can use them to know when she is ovulating, or if she is not ovulating for some reason. At your ages, they usually advise to try to conceive "naturally" for one year before seeking a doctor's further help. But if you've already been trying for a year, maybe they have new advice for you? If intercourse is difficult in any way, you may benefit from doing an IUI, in which you will produce a sperm sample at the clinic that will then be put directly into your wife's uterus with a catheter at the correct time for conception. Did they suggest trying IUIs to you?

As for egg quality, a lot of ladies on this site have had good results with supplements like CoQ10, DHEA, vitamin D, Selenium, Zinc, Myo-inositol, and the amino acid L-arginine. Folic Acid helps with a healthy baby once you are pregnant. Good luck!

Hi, I have had many IUIs and they are a little uncomfortable, like any pelvic exam, but should not be too painful. No pain medicines are usually required.

And I should ad, that a doctor can't know if there is a healthy egg or not unless you have had IVF and tested them. With a scan, they can only see ovarian follicles, not eggs. But maybe her tests indicate that she is not ovulating? A woman over the age of 35 still usually produces many eggs, a number of which are healthy. Did they recommend that you start IVF?

Still they don’t recommend that ivf but told that we may need egg donner in future if all medicine don’t work.. and still just do sonography and told as check size of egg and told the quality next time i try iui and ivf.. i just confused may tight foreskin effect conceive? Ovulation kit we try it doesn’t show result and doctor also one time told us to try 16-18-20-22 days as my wife ovulation is late i think that cause any problem?? In periods of 5 days — 3 days bleeding first day and second dat 2 sanatry pad full and 3 rd day half pad

If you are able to produce semen then it's not a problem. Once your wife's ovulation kit shows that she is ovulating, you should have as much intercourse as possible for three days. If she is not getting a positive result, then she is not ovulating. Sounds like you may want to find a better doctor who isn't going to scare you about donor eggs when they don't really seem to know too much about your situation. The length of her menstrual cycle within a normal range does not effect when she is fertile. Just start taking an ovulation test starting from the 5th day after she first gets her period. Keep taking it until it shows a positive for ovulation, or until she gets her period again. This will tell you when she is ovulating, or if she is not ovulating at all. If she is not ovulating, the next step is usually to try lifestyle changes and some drugs.

Thank you very much for great support and clear all doubts now we try by testing ovulation kit and find ovulation date if ovulation doesn’t show positive and she get in period then what is means??

If she never gets a positive ovulation indicator, tell this to a fertility doctor. If you are not seeing someone at a specific fertility clinic, you should get a referral to see one. You can keep monitoring her ovulation to see if she is always ovulating, never ovulating, or only ovulating sometimes.

Hi Harsh_patel

Luxfluer has given you some really great advice here and I wanted to confirm and reassure you that there are many many tests and processes you can go through before even beginning to think about donor egg or IVF.

It is impossible to tell egg quality unless you have actually been through ivf, there is one test called an AMH test which can give an indication of egg reserves (e.g high or low) but its is not an exact science and it still cannot tell you the quality of the eggs. You could have a low amh and high quality or a high amh and low quality eggs.

Generally the first thing a fertilty doctor will do is run blood tests / sperm test and cycle monitoring to ensure ovulation / sperm quality... Next would be a test to ensure the tubes are open and check the uterus. Providing all of these are fine you would then make the decision of continuing trying naturally for a period of time or using assisted methods. E.g iui, ivf etc. If they do find any abnormalities on the tests then they would try to resolve these before going to ivf if they can be resolved of course.

There are a few really good books avalible to explain more of this and I particularly recommend Zita West: guide to fertilty and assisted conception.

All the best for the future :)

Hello Harsh_patel, my heart goes to you. My honest advice would be"please change your doctor." I highly recommend Dr. Anil Gudi in Fertility Plus. Its in Harley Street. Google him & see the reviews. He's the best in this field doesn'twaste time. He listens patients concerns & provides bespoke advice.

Hey there,Please don’t fret just yet on this - I know how tough it is to hear news like this, especially at such a young age.

I agree with the advice below - get a second opinion with a different Doctor - someone who specialises in fertility. Look at the diet, supplements and other treatments that you can do at home to help.

There are certainly lots of solutions out there that the doctor may want to try before even going down the route of IVF or Donor eggs. Please don’t let this consume you - until you know what your options are.

It’s really great to see you on here reaching out for advice and supporting your wife. Have a look at ways to keep your stress levels down, as you navigate through this process - whether that’s Acupuncture, yoga, meditation etc. But don’t panic just yet - sometimes these things work out quicker for people. Stay tough 🌺
