Hi, I have this question and looked everywhere on net for a grading system which gives a blastocyst 1 as being the top but it also has letters like gardner system as aa, ab, bb, etc.
Ours were graded as 1ab, 2bb for first cycle and second cycle only one blastocyst with 2bb grading.
I was upset until I found through nurse that these are good embryos. They only said 1 & 2 are really good with 1 being the top.
So I believe aa, ab,bb ..etc. Should mean similar to garner grading and also read thT so many clinics use 1 for top quality embryo for day 3 in terms of fragmentation and 1 means no fragmentation.
But cannot figure out what the combination might mean and why they may use day 3 grading for day 5 blastocysts?!!
Anyone having similar experience or know what type of naming is this?

Thanks Sarah for replying to me. I looked at the link but it’s mostly the same things I found online about the Gardner grading and day 3 grading.
None of the systems apply to our embryos these to me are a weird combination of numbers and letters which does not fall under either of categories I read about so far.
I am looking for some explanation for this specific grading my clinic uses, apparently 1 means a top quality embryo but not sure what exactly means , is it fragmentation or blastocyst expansion.
Also being an AB or BB, and what it means . I assume this aa bb might mean same as Gardner system about fetus and placenta formation but would like someone who is familiar with grading to confirm.
I think the best thing for you to do would be to speak to an embryologist at your clinic. They will be best placed to explain the grading system they use and how that reflects on the embryos you have. Answers you get here may cause you undue stress and worry if their embryos have been graded differently to yours. I hope you get the answers you’re looking for x
Thank you very much for taking time to reply. The lab is not available to talk and explain, they said the only way to get a report is through doctor and she is not very reachable so I have to wait until I see her . Which wud be only does when I am there for my next ivf cycle monitoring which is kinda late as I want to get answers and decide based on that. So I try my chances on net until then, but this grading is totally different from all I found and read si far! Even my previous clinic had Gardner grading system , this is different!
Great article, thank you very much mishslade!I believe like you said mine should be very similar to Gardner but could not find any version of Gardner in which 1 is the best. So I am not sure if this #1 refers to blastocyst expansion or no fragmentation.
However I liked your article as it is very detailed, reading so many websites nowhere else I saw mentioning of Symmetry separate from Fragmentation so I learned stuff from your page! Thanks!
As per my embryo grading There is only one website listing the results of some research and they have one diagram showing as 1AA being the best blastocyst but could mot find more details on what type of grading they name them after. Hi I had 5 day blasts the higher grade blasts ended in mmc but my last little weakest blast which was a 5BC is now my daughter The number is the day the first letter is embryo quality and the last letter is placenta quality
If they are able to freeze the embryo then it’s good enough to turn into a baby I was told , it worked for me

I couldn’t sleep thinking that our last embryo wouldn’t make it etc but yes I now know that the grading is a guide only My little miracle was the last embryo we used and thank god we went for it because I couldn’t imagine life without our daughter
A specialist told me that ugly looking embryos can make the most beautiful babies and he wasn’t wrong
The letters signify an assessment of the inner cell mass and the trophectoderm. The Inner cell mass is attributed a grade between A- E and the outer mass of cells are graded A-C.
