Sensitive - end of sperm storage - recommendations for notary in the UK (Leeds/ West Yorkshire)

Hello,

I had donor egg IVF in Prague with my partner's sperm. I think I'll continue to store the embryos (emotional attachment) and consent to discontinued sperm storage.

To do this, I think I need a notary for the consent form. I'm not sure who can do this. Does anybody have any recommendations for a notary in the UK?? I'm based in Leeds, West Yorkshire.

Also, do you think it would be possible to get the consent forms for both sperm and embryos signed now and just 'hand in' the form for discontinuation of sperm storage now and the signed form (dated this year) for embryo storage next year?
