When you do eat after fasting; what window of time counts as a 'meal'?

When you do eat after fasting; what window of time counts as a meal?

From an insulin perspective, how long between courses can the body cope with a meal being, before it becomes, say 3 x insulin hits?

Just tagging Hidden as insulin is his area of expertise.

(Refined) Carbohydrate raises Insulin for hours.

OMAD is normally thought to be 23:1... but I normally fast 20:4, and I think any snack I eat between breakfast and lunch would be insignificant - but an evening snack would break the fast.
