Why has my 3 year old stopped asking to go to the toilet and why does he hide to poo?

My 3 year old has been suffering from constipation since around 8 months of age and is on Movicol. He also has a kidney issue. He was potty trained for wee but has always struggled when it comes to poo. In June he had major Surgery to cure his kidney issues and had a catheter for over a week after his surgery. Since then, he has stopped asking to go for a wee. If I take him regularly he will wee but sometimes he just sets himself and gets upset that his clothes are wet. As for poo, his movicol doseage was increased due to the ongoing constipation. Since it was increased he will hide to do a poo. We have got him to the toilet once and he was excited to hear the poo plop and flush it away but other than that once occasion he has no interest in pooing on the potty or toilet, rewards such as stickers and toys have no impact as all. When he does soil in his pants, he doesn’t seem to care, unless I notice it, he won’t say anything. But as soon as I notice, he will say he is sorry he didn’t I on the potty/toilet and asks for clean clothes. What can I do? His Nanny looks after him one day per week when I work and she wants to put nappies on him, she says he isn’t potty trained. Any help or advice gladly accepted!
