Problems with poo since birth...

Hi, I’m new to this group and would love any advice! In a nut shell my now 3 year old has struggled to poo since birth. I first took him to the GP at 6 weeks and went every week for the next 6 until he was referred. He was admitted when he was four months over night and given a wash out and I was told his bowel was enlarged. He was breast fed until he was 1. He eats well and drinks lots of water. Since then he has been on lactulose, senna, movicol and we’ve been given picosulfate to use once a week if he hasn’t been. His tummy has always been distended and last year he was put to sleep to be emptied out. He’s been given enemas at his consultant appointments too. We’ve also been prescribed suppositories and, more recently, enemas to use at home. He’s now toilet trained for wees and I thought we had sorted the poo too but he’s soiling multiple times a day. My problem is that things are so inconsistent and I can’t seem to get the dosages of medication balanced correctly. I know he had a mass of poo as I could feel it in his tummy and when he sat on the loo, a thin soft poo seemed to be coming round the mass I assume? We ended up giving him an enema tonight and he literally filled the toilet. The poo appears to be soft and smooth, not hard and dry but with all the medication he’s on I should hope it would be soft! The other problem is doing the enema is hugely distressing for him (and us). The GP won’t offer advice as he’s under a consultant and I’m waiting a call back from the consultant. Any one had similar experiences and got to the bottom of it?! (Pardon the pun!) Thanks in advance.
