Got my letter to get my first smear test I am 24 years old. My mum abnormal cells a few times so i know it is very important.
However, when having an examination the smallest speculum is the only one that fits me and hurts the least. Can I have a smallest speculum for a smear test? also does having endo make the pain worse I am mega sensitive and have pain trauma.
Any tips be very much appreciated.

The smallest speculum shouldn't be a problem for a smear test. I think the more important thing is communication with the doctor. It shouldn't make endo pain worse, but it also isn't a painless experience. Have you tried pelvic floor therapy? I was recently sent for a similar problem.
My main advice though is- ibuprofen before. Make sure to take it long enough ahead of time that it will be working. And heating pad after. If you have a portable heating pad, bring it.
At the end of the day, a good, understanding doctor is a great thing to have, but a pap smear also isn't the scariest thing. It's just built up to be so scary. It is uncomfortable, but honestly, with endo and pain trauma, you're probably a lot stronger than you think and you just need to remember that. Also bring a support person if you need them.
