Hi, I am new to CKD support but hoping someone could advise. I take Methotrexate, Naproxen and Omeprazole for R.A. Because of this my bloods are monitored monthly.
I have been feeling seriously fatigued for the last two years and made it my mission to find out what is wrong with me as the doctors don't seem to care. My latest bloods show
20.3.18 serum creatinine was 64umol/L (46-92) This has been(Nov 17) as low as 54 while GFR was then>90
20.3.18 GFR 80 ml/min/173m2
My Gp in November 17 agreed to put me on a trial for 12 months of B12 as he agreed I was borderline at the time. Since then myRDW has come down from 18% to 14.9% (11-14.8). Another doctor in the practice has since stopped these injections as my B12 has now risen to what he calles "enough for 2 years) Doh!
What I am seriously worried about is the fact that I informed same doctor I had a private mma test as well as a urinary creatinine count which came back abnormal at 0.45umol/L. Same doc. stated "it's the serum creatinine we go by, not urinary. Serum creatinine looks fine" Having done some serious research on internet I feel that this urinary creatinine level is dangerously low and could indicate Kidney disease. As I am taking drugs that can interfere with kidneys, please can anyone advise me what to do next? Thank you

Try using drugs.com and see if any issues are mentioned with your health concerns. Sometimes we have to change meds, sometimes a different dosage will work and sometimes you may have to take a particular med to deal with a more severe situation and hope that any minor issues aren't badly compromised.
Thank you for your reply. I already know the drugs mentioned are not good for the kidneys, which is why I am being monitored every month. I was really looking for an answer reference the very low urinary creatinine.
I understood that both types of creatinine test are important. I believe they can use one to confirm the results of the other. Having said that, whereas I have frequent serum creatinine tests, I can only remember having one urine creatinine test over the years. The serum test is probably more popular being more straightforward. I understand that because urine creatinine in a single urine test can be so dependent on various external factors (food eaten, liquid drunk) it is necessary to collect urine saved over 24 hours, so a more involved test. If Naproxen is a NSAID, then that should be avoided - another NSAID together with an undiagnosed/untreated inflammatory illness caused a reduction in my kidney function leading to CKD. Make sure they monitor your function regularly to ensure that the downward trend in not continuing but stable.
Naproxen and Omeprazole are known to cause kidney problems. NSAIDs such as Naproxen are bad for the kidneys and liver. Omeprazole has been found in long term studies to cause kidney disease. Low creatinine in the urine often is caused by the liver which may be a side effect of methotrexate. Have you had your liver function monitored regularly? The combo of Naproxen and Methotrexate could be something to watch in my opinion.
