"We report a case of successful RBD management with prazosin in a patient in whom high-dose melatonin was ineffective. Although there was no observable reduction in dream-enactment behaviors with high-dose melatonin, the possibility of a synergistic effect of prazosin combined with melatonin cannot be ruled out. This case report supports further evaluation of prazosin as a potential therapeutic for RBD."
They used melatonin 9 mg + prazosin 5 mg at bedtime.
Prazosin is an alpha-blocker used to treat hypertension, enlarged prostate, and nightmares related to PTSD.
What is interesting is that terazosin, another alpha-blocker, is studied in Parkinson's with a trial about to start in the UK (EJS ACT-PD): isrctn.com/ISRCTN17799294
