Any suggestions for end of day medication wear off?

Hi all, I’m 7 years post diagnosis and about 5 years into Carbidopa Levodopa medication. I am fortunate that I don’t need c/l to sleep so I generally don’t take any meds after dinner. The final wear off lasts anywhere from 30 to 60 minutes when I experience more severe symptoms and dyskinesia (I don’t have dyskinesia at other times). I usually sit with my feet up and try to relax. Anyone else have any suggestions that have worked for them? Thanks!

Carbidopa Levodopa immediate release version is usually prescribed by default. An extended release version for your final dose may work better. Further detail here: healthunlocked.com/cure-par...

Thanks, park_bear. I have been avoiding the extended release as if it is the “next step” and that would be admitting defeat, lol. But is just makes sense at least for the final dose Thank you for the info. The chart is very helpful.

My HWP dx 9 years has just started an extended release over night so takes it just before bed.

It seems to help in the morning as well as the night in that his body doesn’t start from zero meds in the morning.

I d been asking for a couple of years for it but eventually the PD nurse allowed it and it has helped with dyskinesia generally not eliminated it but seems to not be so frequent.

He’s still on low meds though just more spread out.

try Apomorfin (0.2-0.4 ml, subcutaneously on the belly)

I usually pop a few mucuna pruriens extract capsules to smooth out the transition period. You can buy them online from amazon. I have been doing this for over five years with no ill effects.
