I’m on W3 and should be on my last run but I am struggling to manage a full 3 minute run. Should I just repeat the week again?

Your wording is imprecise as to whether you did or did not complete the workout. The advice in the guide to the plan healthunlocked.com/couchto5... is that if you do not complete a run then you repeat that run, not necessarily the whole week.
If you complete it is recommended that you move on. You may surprise yourself, but you will need to pace yourself. Slow is good.
