CLL Society Expert Access™ Program

application: cllsociety.org/2019-expert-...

-

We believe that access to CLL expert care is critical for every patient to receive his or her best possible care. Access to a CLL expert provides proven survival and outcomes advantage. In other words, those with a CLL expert as part of their team, live longer than those without this advantage. This program offers patients access to CLL expertise through a HIPAA compliant online video consultation.

-

The Process

Step 1: Confirm that you Qualify

Click this “Get Started” link: cllsociety.org/2019-expert-...

to attest that you meet the criteria. To participate in the Expert Access Program, you must reside in the United States and have a CLL diagnosis, without current access to a CLL expert. If you are unsure if you are already seeing a CLL expert physician, check our list of CLL expert doctors here.

Step 2: Pre-Consultation Survey and Medical Record Release Form

After qualifying for the program, you will be provided a link that takes you to a DocuSign form. Completion of this form is necessary to begin the program. The form includes:

-

Pre-Consultation survey.

Your responses to the Pre-Consultation survey will only be reported in combination with all other participants in the program and will not be shared individually.

Medical Record Release Form.Upon completion, you will receive a copy of the Medical Release form for your records. In preparation for your consultation, your medical records will be summarized and provided to the CLL Expert prior to your consultation. Your medical records will be kept confidential and will not be shared outside the scope of this program.

Again, all information must be completed in order to begin the program.

--

Once you complete the DocuSign form, you will hear from our partner, InfiniteMD via the email careteam@infinitemd.com.

-

Step 3: Medical Records Collection

One of the InfiniteMD case managers will reach out to your primary oncologist to collect all of the necessary medical records for the CLL Expert to review. This may take up to 10 days.

-

Step 4: Scheduling and 30 minute video consultation with a top specialist

Once InfiniteMD obtains your medical records, and summarizes your case, they will send you a few potential time slots to choose from. A 30-minute video consultation will then occur between you and a top CLL specialist. The InfiniteMD platform is very easy to use and available on most internet connected devices. It is preferred to use a device that has a camera to facilitate the video consultation. InfiniteMD can offer tech support if necessary.

-

Step 5: Post-Consultation Survey and Consultation Summary

Following the consultation, you will be asked to complete a brief survey to help us evaluate the effect of the program. Your survey results will be aggregated with those of other patients and will never be reported individually. You will also receive a summary of your consultation written by the CLL Expert, which you can then share with your treating physician and keep for your own records.

-

Our Partner

CLL Society has partnered with InfiniteMD, an online second opinion service that uses live, synchronous video technology to match patients around the world with top US-based medical specialists. Eighty percent of those provided a 2ndopinion through InfiniteMD were seeking oncology consultations.

Our Supporters

The 2020 CLL Society Expert Access Program is made possible by support from Verastem Oncology (Expert Access Founding Supporter) and Genentech.

I have used this twice, it is very helpful, especially for someone who lives in the "outer territories" - Hawaii

Take advantage of this, always great to get other opinions from CLL experts with up to date knowledge.

Totally wish we had this available in Canada

Is there an option that would prevent your current doctor from knowing you sought a Second opinion or do they always contact your current physician? I still wonder why doc is always so ready to treat with normal labs and quality of life when Gazyva alone does wonders whenever the time arrives with literally zero side effects.

In order for the CLL Expert to have the required information about your medical history and blood test results, they must get copies of your medical records. These are ususally obtained by submitting a formal document to whatever entity has those records. If it is a hospital, your doctor probably will not know. If it is a private practice, the staff may consult the doctor about releasing the records.

-

But, we have said many times, it is your life and your medical history, you have much more at risk than the doctor does. So if the doctor is unprofessional or hurt by your attempt to live longer, then it is unfortunate.

my doctors are using a medical portal called MyChart where it shows the diagnosis. Can I send those documents instead of my providers getting contacted?

My understanding is that your doctor needs to be in the loop, and any records needs to be generated through the doctors office. If your doc is upset/unhappy that a renowned specialist in this rare cancer is being asked for input/opinion, that's something you need to know.

I agree with SofiaDeo , we get many patients that fear anger or retribution if they ask for a 2nd opinion from a CLL expert. I won't guarantee that it cannot happen, but if it does, you will be much better off (and probably live longer) without that doctor.

You will be much better off having a local doctor that is a good diagnostician that can deal with routine infections, mystery symptoms and other things that can kill us immune compromised patients,. And then that doctor & patient know when to consult a CLL expert to recommend when and how to treat the CLL.
