Has anyone been put on a modified (rapid ramp up) venetoclax plan? Edit: My Dad has completed the first full week on 20mg but will now cut the 50mg week short to 4 days -proceeding to 100mg on the 5th day.
Does anyone have experience with a similar or different modified dose ramp up? Thanks.

I have the meds but specialist suggested I wait a couple weeks since I’ve been sick, I’m now much better and will see him Tuesday. Could you share the new modified ramp up?
Thank you cllady01 for your message. I’ve looked through similar posts but wanted to post in case I had missed something or someone might have had some recent experience they might like to share.
When I looked back at your original posting I found this information: My Dad was diagnosed with TPLL in 2013 so you are trying to transfer our experience with Venetoclax on CLL to another NHL - TPLL
I think our best advice is to make certain your Dad is closely monitored by a expert doctor familiar with Venetoclax, after each dose increase. He may need blood draws and "STAT" test results before each dose increase and at 4-6 hours, 8-12 hours and 24 hours after the dose increase.
In my case I was an inpatient for 30 hours for the 20 mg and 50 mg doses, and when the 24 hour results were interpreted, then I went home to take the remaining 6 doses.
I took the increase to 100 mg, 200 mg and 400 mg, as an outpatient with a much more relaxed schedule of blood draws, so I assume that your doctor and mine saw the biggest risks at 20mg and 50 mg.
I assume your Dad's doctor has a specific reason to shorten the time between the 50 mg and 100 mg dose.
Hi lankisterguy, thanks for your message. I appreciate your advice but I am not trying to transfer CLL experience to my Dad’s case. I am sure I had seen another person’s post about tpll here hence thought it would be okay to post here too in the hope of getting a further general understanding of venetoclax as I have not yet come
across a similar medium/forum for tpll. The same protocol of ramp up has been initially applied in my Dad’s instance and I was hoping that nothing has been missed. I understand these are completely different leukemias. Appreciate your message, info and advice.
I'm sorry if you were upset or offended by my mentioning the TPLL and the experience transfer, I was not implying that your request was inappropriate or unwelcome. I was only trying to make all the background info available to other readers.
I am very happy to share my experience with you- as you can see from my reply. I hope it is helpful to you and your dad. And please ask if there are other details we can share.
I know I saw someone post about the faster ramp up somewhere - maybe on cllforum.com/, and maybe at Ohio State. You might try posting your question there.
