Sorry it has taken a while to get back, So the phone call with the consultant was frustrating and disappointing to be honest.
He started by mentioning the "very serious infection" I got the feeling that he thought I didn't know what it was, I said "yes, the Spontaneous Bacterial Peritonitis" I asked if it was likely to happen again, he replied "yes" I then asked about taking a low dose of anti-biotics as a prophylaxis, he said "yes, that would be a good idea..." How will I know if I have SBP, I had NO symptoms.
I asked about my prognosis, he answered by saying "your kidney and liver function tests are almost normal"
I asked how could I have gone from NAFLD to Cirrhosis in 9years without knowing! He replied that it "usually takes 30years to go from NAFLD to Cirrhosis, that me having type 2diabetes, being overweight and having high blood pressure, plus high cholesterol/triglycerides, saying I had Metabolic Syndrome (1st time mentioned to me) have all contributed to hasten the decline!
1. Only Dx'd with type 2diabetes 3 yrs ago.
2. Blood pressure only elevated during Hyperparathyoid phase, controlled well since.
3.High Cholesterol and Triglycerides, yes, off the charts before medication, cholesterol now controlled but triglycerides still high. I explained to him that both of my sisters (healthy, normal weight etc) also have high cholesterol and that our father died aged 47 from a massive heart attack, that the cholesterol might be a genetic issue, he agreed and said there could be something in that!
He asked if I had always been overweight, I told him no, that since the Hyperparathyoidism I have been on many different tablets, some causing weight gain! (20 different meds, around 40 ish a day).
He asked if I had ever been seen at the Liver clinic? Answer NO!
He is now arranging another CT scan, I have a blood test next Thurs, when he gets the results of them he will arrange another phone consult!
I asked "what if I think the Ascites is coming back or if I feel ill, WHAT do I do?"
He replied " come in, either via GP, out of hours or A&E"
As for the prophylaxis of anti-biotics, he changed his mind, because bloods ok I don't need anti-biotics!
Any literature you read about Cirrhosis, AKI and Spontaneous Bacterial Peritonitis happening together states the patient should immediately be referred for assessment to go on the Liver transplant list! If a person survives SBP they should be on prophylaxis anti-b's.
So sorry it is so long again, I am frustrated & scared, maybe I expected too much.
