Really interesting reading your replies. I have just been advised to go on to Entresto lower dose am and higher dose pm. This was due to low BP readings under 100. Will know my blood test results soon to decide whether I can tolerate higher dose twice a day. I have had a CABG and mitral valve repair (2017). Recent EF at 34% (seems to be a general number that keeps occurring in these posts). I was told that an MRI is not possible with an ICD fitted. Is that true???? Did get the dizziness when standing up quickly as I adjusted to the drug, but that has not been so evident recently. Personally, I will be happy when the surgeon tells me there is an improvement in my EF! Wishing you and all those who have posted the very best. Hope the price of Entresto comes down soon!

I’m on lowest dose 24/26 in morning and the 49/51 (I think) at night does to same issue
It was an interesting discussion thread wasn't it? Seems like this is a brilliant drug for those who can tolerate it. But as commented, soooo expensive hence restricted access. Lets hope price comes down soon as GP was reluctant to take over prescribing, but eventually agreed to do so. I was recently put on Entresto after nearly two years of trying to stabilise my heart function following complicated second AVR. So far so good, although it really does knock my BP for six early morning. Yesterday 88/54 - but it does come up after a glass or two of water and a couple of hours. Its ok for rest of day. Waiting for results of kidney function bloods to see if dose can be increased - although with BP that low maybe not. Cardiologist wants to give it a couple of months so that some habituation can take place. Even though early days, I have noticed some slight increase in my exercise tolerance and I;m less fatigued generally. Hasn't really done much for breathlesness = yet, but I'm hoping that will follow.
Good luck taking it -
If you have one of the newer ICD's fitted these are MRI compatible. If you have an older ICD then some hospitals are now able to do MRI's subject to certain precautions being taken, but as my husband's cardiologist said you may have to travel some distance to find a hospital. I know of someone with an older ICD who recently had a MRI at Papworth & Bart's do them for my husband's make & model of ICD. Of those hospitals that do them they don't all do the same make's & models ( if that make sense).Your cardiologist should know which hospitals can now do them, If not they need to start researching! As my husband's cardiologist said " don't let anyone ever tell you you can't have an MRI".
