having been diagnosed with Heart Failure last year I was put on revised meds (Entresto, Dapagliflozin, spironolactone), symptoms, improved. After a few months Consultant increased dose of Entresto to the middle dose. Ejection factor had improved from 20/25%, to 39%. . Also replaced Spiro with eplernone due to unwanted side effects.
Since these changes I have struggled with fatigue and tiredness, suspect the higher dose of Entresto might be to blame. Anyone experienced this problem?

My husbands on entresto and finds it helps his energy levels. Are you on any other meds like statins or beta-blockers? Heart failure can cause tiredness in general. Have you had your iron and haemoglobin levels tested recently? A lot of heart failure patients have low iron levels, my husband has had to have several transfusions and takes a multivitamin as well as co-enzyme q10 as this is supposed to help with energy. I’d recommend speaking to your GP and doing your own research before taking any supplements incase they interact with the medication you are taking.
Hiya,have just happened across your post…I’m about to have my Entresto dose increased so will be interested to see if it has an effect on my fatigue levels.I too am on the combination of spiralactone and dapagliflozin….am really happy with the results though after my initial reluctance to take so many meds altogether my ejection fraction has gone up from 30-35% to 35-40%❤️So good news and have been told I won’t now need a defibrillator
Hello. Yes 100% entresto causes extreme fatigue and brain fog. Tired. Even within 30 minutes of taking i used to feel this cloud of 'urggghhh' coming over me. My brain went to mush. Tired. Aniexity sometimes kicked in. Felt crap!! So, i used to split the tablets in half and spread them out throughout the day. That kinda worked. This happened for 2 years on the middle dose. My ef went up initially on lower dose but it dropped slightly on middle.
I have however stopped getting side effects so much... Finally, after 2 full years on middle dose. Actually since taking dapagliflozin, that seems to have made me feel slightly better from those sides. Vanished.
But i still do get episodes of bad aniexity. Spaced out floating feeling. Sharp chest pains. Low bp and have a slow heart rate of 60-65 most of the time. Dapa seems to slow down things since taking it. Just hoping the scan next month shows some improvement.
Although your scan says heart has improved... Have you actually improved in some way? Symptoms not so bad?I went for months and months thinking entresto was making me worse. That the drugs side effects are the only effects i am getting! I honestly thought about stopping the lot. But powered through form hearing all those peoples benefits from it. So will see. Ill update if any improvement from the dapa. Started it jan this year and ef was 35-40%.
Yes that is happening to me too.I increased the dose to middle dose nearly three weeks ago. But only in the evening. In the morning I am still taking the low dose. I am getting very weary and droopy (But I will be 80 very soon !)
I don’t know whether I should give up on Entresto and feel livelier or persist and live longer. Hobson’s choice !
