Hello and welcome to the forum! At a guess you are probably better insuring separately. The longer the period of "good" health from the fitting of your ICD the lower the premium. If it is for your holiday in Barbados premium will be high as classed with US.
It will probably work out cheaper for you to get separate insurance policies- use a comparison site and you’ll get an idea as to what the difference might be. My wife has health issues that are different to my own - she can only get single trip insurance whereas I can get annual insurance for less than her quote. For the Caribbean it will be the medical cover that will be the critical bit of the policy and will push the price up.
If you insure separately and you are taken I'll so that early repatriation is required, or an extended stay in your holiday destination, I'm not sure whether the additional costs of your wife for early repatriation or an extended stay will be covered.
Hi I needed special insurance following cardiac arrest and my wife had previous TIA. My suggestion is try as many insurers as possible as they all quote differently , we ended up with Avanti but give them all a try

I watched a man being winched off a cruise ship off the coast of Oman. His policy covered him . His wife had a cheaper policy and she had to stay on the ship until it reached the next port, which was Dubai, where she went ashore and had to manage from there on .
I found Staysure was very good, I agree about both having the same insurance the above post shows a cautionary tale.
Try Flexicover. Been with them 3 years. Annual cover for husband Aortic Valve Replacement and myself. Each medical condition has an additional valve onto of standard cover. Be aware that travel cover to USA Canada and Caribbean costs more because repatriation is dearer.
