I’m sitting in Spain today ( bright sunshine and frost - lovely ) and idly checked my NHS record.
I was interested to see on my record that today while I was climbing on a mountain I had had a consultation at my surgery and my Creatine Clearance had been calculated without any notification, without my physical presence nor any of my blood or urine which is plainly miraculous or I’m missing something….,(and I’m always pleased to be educated).
I see that using the Cockcroft-Gault formula my estimated Creatine Cleareance is 94.364 and my dosage of Edoxaban is unchanged. It must be based on previous weight, age data etc … my last bloods were over 8 months ago well before I started Edoxaban.
Yes me! My last bloods prior to this were in Feb 22. Thanks for sharing. No one has been in touch so presume ok??
Found this on National kidney foundation website "The Cockcroft-Gault (CG) formula has not been expressed using standardized creatinine values. This means it will give inaccurate results. It is not recommended for clinical use."
it would appear okay so no one has been in touch - before, during or after. As it’s only a calculation anyway there’s nothing to say …. no one would know what is the real reading …

I haven’t had any blood tests for months so sure this doesn’t relate to me. Waste of time calling my surgery because the receptionists just haven’t got a clue.
I have blood tests every 12 weeks so I have a consistent record of Creatine levels - so in my case I see the logic as I’m also on powerful drugs which take a toll.
I understand - but you have blood tests. Mine was based on old data from before I started in the medication. Mine was a calculation based on something historical…that’s what I don’t comprehend.
I’m not arguing against the need I just want to understand the method- calculate without current samples ?
thanks ….I’m going to the surgery to ask the question. They’re usually very helpful so I expect to find out what lies behind this method and I’ll post my discovery.
I went into my NHS GP notes the other day to order my repeat prescription and as usual checked any updated notes - low and behold a coded entry re My estimated creatinine clearance level (Cockcroft Gault formula)
It looks to me like a ✅ in the box exercise for GPS that earns them brownie points !
No one has contacted me so am presuming my result is within normal limits !
Yes. I imagine this is an exercise to find a statistic for some one to use and could be compared with something else later on - maybe next week if you go into your records you will find something else has been done to be compared with these figures. In the first instance though like you I would be "concerned" to say the least and wonder if this was supposed to be done on my records or someone next to me alphabetically or NHS number wise who could be waiting for treatment based on these figures or that the operator had not moved off my records to the next record they needed to deal with - believe me this has happened to us in the times when not all this was on computer and we have had letters with results sent to us that were not ours but funnily enough one of us was waiting for so had been sent to the wrong address - luckily we noticed other things mentioned that did not match with our situation and got back to the surgery. In those days just a matter of the wrong address being put on the envelope or the letter being put into the wrong envelope. I shall be interested to hear what your surgery says about this. Please keep us informed.
so the estimation gives a result that’s within normal limits. Like mine I guess. A remote estimation ……
As reduction of kidney function is a quoted risk of Edoxaban I was expecting more that someone just running some numbers…..
Perhaps they might have a different calculation to see if my AF has progressed since last year ….
Funnily enough,I had an 'invitation' to provide GP surgery with my current weight and height so they can ' adjust your medication if necessary' As none of my meds are dependent on those factors and I havent reached the magic age where my apixaban is to halve,I shant bother.I had my appointment with my EP recently ,and some minor adjustments made to water tablets ,otherwise carry on as per.
I have CKD (chronic kidney disease) , and 98 is an excellent, normal eGFR. (eGFR= estimated glomerular filtration rate), usually interpreted as % of normal kidney function. Mine is usually in the 40s and 50s. A lot of people with CKD have eGFR in the 10s and 20s and have to go on dialysis! Or transplant. You're blessed. Don't worry about this, somebody was just double checking your kidneys and didn't find anything abnormal to discuss with you.
Thanks for your response. I’m not actually worried though just very curious about a process that tells me something about me without me being involved in any way.
