Advise on SRT

My dad is looking at a possible bcr. PSA is at .17 15 months post surgery for G8 (4+4), age 74 now. Another test is scheduled next month to confirm the rising trend. For anyone of you who have gone through successful SRT, please advise on the following : 1) How long was ADT prescribed for ? and was it only 1st gen or 2nd gen? Is it started immediately with rt ? 2) did SRT include lymph nodes till aortic bifurcation or just pelvic floor 3) is RT done for 7 weeks at 70 GY or higher 4 ) what kind of exercises are recommended to overcome the effects of ADT 5) any specific RT machine recommended ( example MR linac ?) 6) Any side effects during RT and how to overcome those . In 2007, 36 sessions of IMRT salvage radiation of the local area without any ADT gave me 7.5 years of undetectable PSA.

Back then I waited until my PSA reached 0.20. If I had my current knowledge back then, I might have taken action when my PSA reached 0.12.

ADT has lots of negative side effects. For me, lethargy and losing muscle mass were the worst short term effects. I think it pays to force oneself to try to build/rebuild muscles every day with a minimum of 45-60 minutes of muscle building exercises. I also lost some bone density even though I tried to counter that with supplements.

For my GI system, I cut back slightly on vegetables and ate a little bread each day (which I normally do not do). One Imodium took me from diarrhea to constipation, so starting on a partial dose might be a good idea if he experiences loose bowels.

(You may want to take an alpha-blocker, an NSAID, and daily ED meds prophylactically.)

I did five sessions of SBRT, rather than 30-40 sessions of IMRT. Was very happy with the 5 sessions of SBRT. No side effects of the radiation therapy.
