Low PSMA & Pluvicto related questions

Hello

recently my PSA start increasing and I failed the Xtandi again.

Dr refer me for Pluvicto and Dr nuclear medicine told me I do not have enough PSMA for pluvicto from my previous PSMA PET SCAN

now they are ordering FDG scan to if pluvicto is good options or not.

Questions:

- Does FDG is good steps to decide whether pluvicto is good options or not if PSMA PET SCAN shows lower PSMA Spots ?

- I asked for docetaxel or clinical trial that focus on system therapy however doctor keep pushing for pluvicto. I am not sure how can I convince that pluvicto is not good options for me

Keep advocating for yourself! Get a second opinion from a major cancer center (and ask them about clinical trials). And, get the FDG scan as you can never have too much info.

As I understand it, Pluvicto is now considered a better tolerated treatment than chemo and is the first choice to see if it will have an effect. If it doesn't, then chemo will and the SEs from chemo can be quite something.

I understand that you just want to knock the sh*t out of the cancer right now. Good for you! Now you just need a little more info to convince your MO (or a new MO) to follow your wishes.

A FDG scan will not help to see the PSMA expression. It is the PSMA expression which is necessary for a Pluvicto treatment.
