progressing bone mets under triple therapy?

Hi dear community,

My dad who has been diagnosed with mHSPC and bone mets had his restaging after undergoing triple therapy (check bio for more details). So far we‘ve only seen blood results which looked encouraging with a dropping PSA and alkaline phosphate. However at his bone scan it said that while some mets have regressed, some others remained as they were and some on the pelvis even progressed. The radiologist doing the scan said that apparently chemo + radiotherapy continue to take effect months after finishing treatment, so I‘m now wondering whether this should be a concerning assessment or not?

For context, we are still waiting for PSMA/CT results and a conversation with the treating onco urologist. Also, he used to have a renal catheter as they couldn‘t put one in the bladder (tumor was in the way back when he was diagnosed), but this has now worked

I am super worried that we‘ve been too optimistic after seeing the blood results and the fact that he‘s been feeling so well…

Thank you Tall_Allen and j-o-h-n – the oncologists suggested an off-label use of Pluvicto to target the bone mets. Interesting approach as in the EU (we live in Germany), Pluvicto is not approved for hormone-sensitive prostate cancer. I‘m keeping my fingers crossed that it will do the trick and help keep the disease at bay.
