Hi all, my dad (68) was diagnosed with high volume metastatic stage 4 PCa in November 2024. He was put on Decapeptyl injections every 3 months, and started triplet therapy in January (darolutamide + docetaxel).
I wanted to share some updates and ask for your opinions on his progress:
November 2024: initial PSA of 1,048. Decapeptyl injection + bicalutamide.
January 2025: PSA dropped to 28. Started triplet therapy.
After two docetaxel infusions, PSA dropped to 11.
Alkaline phosphatase is dropping too, but remains at around 400. I love my dad so much. Any thoughts would be greatly appreciated!

Praying for your father and your family that he will continue to have such a positive outcome on his treatment regimen. Your love and support for him will go a long way.
Once the 6 cycles of chemo are finished I imagine that your father will continue to take darolutamide. At the moment in Italy it is not possible to add xofigo or pluvicto together with a second line hormonal drug. When darolutamide fails (I hope as late as possible) you can switch to xofigo or pluvicto.
G's Dad is already off to a great start! And it's possible after chemo the ongoing doublet will be successful for years. Randomly suggesting adding additional science fiction therapies now is just irrelevant noise. Do you have any high quality studies supporting your idea? People embarking on this journey after such a recent diagnosis need focus and good information that supports decision making.
Great response. As far as the alp, that can sometimes take a while because it can actually rise while the bones remodel. The fact that it is coming down is great news. God bless.
