Just had bloodwork done which shows 5 slow consecutive rises in PSA - latest is 0.7, so still very minor. But the predictable continuous rise with a PSA doubling time now of about 6.4 months. Heres my updated graph with PSA on the y axis on a log scale, and time on the X axis with 3 month intervals. The blue dots are PSA measurements except for the last 2 which are prognosis for my next visit, and predicted time to PSA=2.0.
My oncologist suggests that we move from Xtandi to Zytiga and prednisone to see if we can stem the tide. All this appears to be standard for PCa progression and if Zytiga doesnt show an improvement I guess chemotherapy will be next on the list.

OK - I started Xtandi 6 okt 2021 after 2 consecutive rises (the bump in the middle of the curve) and it kept things at a minimum til 10 dec 2023 - so thats a little over 2 years. If you look at the slope on my plot you will see that the first rise is steeper than the present rise so as I see it Xtandi has not failed - its just that the beast has found an alternative (slower) way to progress.
As to dosage (for moosejawg I started with the regular 160mg, and then 120 before going to 80mg. Then in the slow decline I tested 120 for 3 months before going back to 80. Did this again for the last 3 months - back up to 120 - but all of these variations had no effect on the slope og the curve. For me this is significant since the slope of the curve over the last year has allowed me to accurately predict the PSA level 3 months ahead of the blood test.
I'm interested in next steps as well. My PSA is still very low, but it has been at non-detect levels for the last 4 years. Please let us know what path you choose and what your results are.
I am interested in hearing also. My PSA a year ago was less than .01, March and June of 24 it was .01. Aug was .02, now this week it is .05. Doubled in 3 months, even thou it is still very low. I am on xtandi since June of 22. We are going to do a PSMA-Pet scan to start with. Thanks, Keith
I would get a PSMA PET scan to see what’s going on. My cancer didn’t exhibit high PSA…went from undetectable to .41, to .53 then .91. (You can read the whole story in my history.) My scan showed my PCa had fully metastisized. The sooner you find out what’s going on, the ore options you’ll have, IMHO.
Yeah, Im with you there. Fortunately for me my PCA so far is highly sensitive to PSA and I have been able to track the regression and progression quite accurately with the three monthly PSA reading. But sure if it mutates into somethinh which does not show in PSA readings then there will have to be more regular scans. In january we will switch from Ztandi to Zytiga an see if that shows on the curve, but after that it will be some Docetaxel I think - with some new scans.
