Is PSA 0.6 after 2 months ADT but 3000 ALP & below normal RBC PCV Neutrophils & Basophils good ?

I am new here. Diagnosed 4+4 T3bN1M in June 2024. Started ADT in October. After 1st month PSA3.6. After 2nd month PSA 0.6 last week. I was hoping to achieve under 2 after 3 months so I am happy with the result so far. I do take a lot of other things apart from ADT because I don’t want chemo or radiotherapy. My RBC 4.1 and PCV 0.353 are below normal so as my Neutrophils 1.8 and Basophils 0.00 (is that mean I have 0 ? I take Loratadine, is that the reason?). My ALP is very high. Last check was 3000. My doctor said because the PSA is dropping fast so it can be the bone is regenerating. Is anyone experiencing similar things? I’ve been playing drums every day as part of my exercise plus pull up bar and medium weight lifting. Have to be careful because my metastasis.

When dad was diagnosed his ALP was 180. Started orgovyx and after a month psa came down to 1.12 from 18 but ALP rose to 320. MO said that’s ok as most likely bone activity due to the adt. Then started xtandi and the next test showed psa at 0.07 and ALP at 53 - came down fast !

Generally we get a second opinion from a specialist, such a medical oncologist from a reputable cancer hospital. I have read stopping PSA progression, does not mean the end of radiographic progression. (seen in scans), that may show bone mets.

I understand because I have 2 types of cancer cells and one is an unpredictable type. My first bone scan was in June. I’ll need to find out when my next scan is in 3 weeks. I feel no pain so hopefully it’s not a bad news.

This is a journey. Mine started in 2012…..I’ve had chemo a few times and it’s always brought PSA down…except for Pluvicto. The chemo generally extends life but I’ve dealt w fatigue etc. if I had to do it over I would do it again.

3000 sounds very high, but perhaps others can weigh in. Mine did rise initially, but stayed within normal range, then dropped to all-time low within a year or do after starting Lupron.

Chemotherapy. Don't do it. I know personally 3 ex-friends that died less than a year an a half after doing it. So did O.J. If you went my route you would live on as I have for 14 years now.

I had "extensive" bone mets when diagnosed. My ALP went to 1850 shortly after I started ADT and then decreased to normal over a few months. My MO also said it was due to bone repair. That combined with your dropping PSA says that you are responding well to treatment. Be glad about that.
