I had a radical prostatectomy in 2018 with some positive margins and a Gleason 7. At the time it was worrisome for me but my psa was undetectable until May, 2023. Now it has risen to .2, not that high really....I live close to the University of Virginia, and they are wonderful....but they're not current, or at least I don't think they are, with developing research and alternative methodologies. Maybe what I'm really looking for is a PhD in the field whom I can sometimes see remotely instead of traveling all the time. Any suggestions?

Get a PMSA test and get salvage radiation if recommended. You need an oncologist not a PhD.
Johns Hopkins has some of the best oncologist's in the world. IDK why you want an integrative oncologist.
Good question. Maybe I don't want an integrative oncologist. Mostly I don't want to miss out on anything that might reduce the risk of illness or death from prostate cancer. Here at UVA they recommended a psma test, which I am getting in a couple of weeks. If anything shows up, they recommend radiation as a first line treatment. Before any of that happens, I know there are non-invasive diagnostic tests which might help guide treatment, or at least offer alternatives to radiation.
Maybe I am just looking for a researcher who accepts patients, and not an integrative oncologist, but someone with more knowledge of current research developments. Can you suggest what I should be looking for?

Johns Hopkins also operates Sibley Hospital in D.C. and Suburban Hospital in Bethesda, both a bit closer to you than Baltimore. I've been treated for PCa at Sibley for the past 2+ years, and would recommend them without reservation.
oh thank you for that. May I ask who worked with you at PCa Sibley or Suburban Hospital? Yes they are both closer and I have friends who live in Bethesda and the Chevy Chase area.
I called Hopkins (Brady Urological Institute, 443-341-0737) as soon as I got my diagnosis. They sent me to Dr. Gerald Andriole (urologist, might be retired now), who set me up with testing, including MRI and PSMA PET scan, along with second opinion on my biopsy slides. Then I was quickly referred to Drs. Aditya Halthore (radiation oncologist) and Channing Paller (medical oncologist), who have managed my treatment. Sibley also has a proton center, which I opted for. I've been very satisfied with all aspects of my treatment!
IDK why you want an "integrative oncologist". You want someone who handles cases like yours frequently and successfully. If your case is something they cannot handle or they are out of SOC arrows, you need to get into an appropriate trial and hope for the best.
