Assisting my husband with locating a "proactive" medical oncologist in Southern Florida area.
Thus far consulting with, and being treated by a radiation - oncologist.
Metastatic PCa,this is his second recurrence- post initial radiation to prostate in 2012. radiation to mets in mediastinal lymph nodes, 2015. In addition to ADT both times. (not on ADT now.) He is having Gallium Ga 68 labeled PSMA-11 pet/ct scans done this week, but would really like a good med. oncologist to consult with after these scans. Any suggestions would be greatly appreciated. Thank you

Dr. Datolli treated the mediastinal lymphs; we were told at Mike's dx. that this was not an easy area to treat with radiation. It
seemed to be successful ... we will see what the new scans show. Going to the University of Michigan, for the scans on Friday. Have not spoken to Dr. Sylvester at UM but will look up his bio, trying to find someone to consult with after we return. thank you
Sorry - I meant the Sylvester Cancer Center at UM, not Dr Sylvester (who is an RO).
The issue with mediastinal lymph nodes is not whether it can shrink them, the issue is safety. There are long-term deleterious effects of low levels of radiation on the heart muscle, the lungs, the esophagus, etc. Most ROs I've met refuse to irradiate those, especially because there is no known benefit, and high risk.
I hope this is not the case, but time will tell. we had heard this prior to his radition in 2015. I don't know if Dr. D has treated other men with mets to m.s. lymphnodes?
We are planning on making an appointment with Dr. KA Ahmed at Moffitt in Tampa because he has authored several papers on SBRT for oligometastatic prostate cancer (example below). We haven't seen him yet.

We go to Dr Negret at Sylvester. He is on his game. We love him. He replies to all questions and emails. He is compassionate and caring and very bright. He has our next plan of action on the table when we walk through the door. He is aggressive and he thinks outside the box. We had a Dr at Miami Cancer Institute who
Was non communicative and did protocol and we felt like we wanted a more caring Dr who was into his job. Dr Avitol is the R/O we went to
For xofigo at MCI and he is brilliant. But we love Dr Negret med onc at Sylvester now
