My husband's PSA count is 3631. He's had every treatment bar Lutetium and immunotherapy and he doesn't have the BRCA gene. Are we at EOL? He, although emaciated, believes that it's not over yet.

Why was Zytiga a "no-no"?
He found (thought, because he'd had a really bad response to the post-Chemo steroid shot in 2022) that the Prednisone/Prednisolone was making him unwell - gastric pain and the start of really nasty chronic constipation which was not addressed. He opted out in Nov. We are in Ireland and he's a public patient (although the Oncology care is superb most of the time, when he began Cavotaxel last June and mentioned this handicap he was prescribed Oxycontin - he hasn't taken it but has fallen into the trap of NSAIDs for pain relief). He's now on Tramadol and, although spaced, feeling better. Thanks for your rapid response.
I would just like to say that I feel your pain. I'm in a similar situation with my father, but my dad has long ago accepted the situation and is not keen on going through aggressive treatments to potentially prolong life (my dad will be 82, but besides cancer, he is in excellent health).
If your husband is willing to fight, than I would get second opinion from most reputable oncologist in Ireland / UK, unless you are willing to travel to India, Germany etc, then you could also contact them. As Allen says, what about Zytiga or Xtandi? It seems that most men here tolerate Zytiga better then Xtandi.

Is he too weak for Lutetium and is it available with your coverage? It can be real tough on you but maybe? I have high hopes for LU in my case but I was pretty robust when I started and am tolerating it well.
We are in a similar situation. We are devastated. I am sorry this is happening.
It truly is devastating. Both emotionally as well as physically. I tell to myself I can't break down now so I should not let my mind start with "what ifs" etc. I have to be strong for my father (I am his only caregiver). I must reassure him I will be ok. And all I can do is love him, be with him and make sure, that pain (when is comes) is well managed.
That's a tough one. In Dr. Michael Greger, M.D.'s book, "How Not to Die", (2015) he recommends ground flaxseed to reduce proliferation and thwart the progression of prostate cancer PSA and tumors. "Why not give it a try?", he asks. I personally would recommend frequent heat such as saunas, and lycopene consumption. You might try that also.
My husband is about the same, emaciated, has had all treatments now to start Pluvicto however not sure he can handle it. Had fluid in lungs, now 2 catheters, psa 2000, no energy, constipation, and just so depressed. Oncologist talked hospice which really devastated him. He has been fighting this monster since 2011 when he was only 51 years old. We are both thinking this is the end. He had chemo and handled it well, on 2 clinical trials, kept mets stable but psa still rose substantially. Good luck to you judym
If your MO gives the OK for Pluvicto it might help, I have just completed 6 Pluvicto treatments and side effects are less than chemo. Hopefully your blood labs and scans show that this is an option.
Also, I have found that a well sourced plant diet is benefical.
